经尿道电切术治疗对膀胱肿瘤患者疗效及并发症的影响

Efficacy and Complications of Transurethral Resection of Bladder Tumor

目的:探讨经尿道电切术治疗对膀胱肿瘤患者疗效及并发症的影响。方法:选取2012年1月-2014年2月本院收治的膀胱肿瘤患者88例作为研究对象,依据随机分配原则分为开腹组和电切组,每组44例。开腹组患者给予常规开腹肿瘤切除术治疗,电切组患者给予经尿道电切术治疗,随访1年,比较两组患者手术时间、术中出血量、住院时间、尿管留置时间、并发症情况及术后1、3、6、12个月的复发情况。结果:电切组患者手术时间、术中出血量、住院时间及尿管留置时间均明显低于开腹组,比较差异均有统计学意义(P<0.05);电切组患者并发症发生率明显低于开腹组,比较差异有统计学意义(P<0.05);电切组患者术后1、3、6、12个月的累积复发率均明显低于开腹组,比较差异均有统计学意义(P<0.05)。结论:经尿道电切术治疗可有效缩短手术时间,减少对膀胱肿瘤患者的创伤,降低术后并发症的发生,有利于减少复发的发生及提高临床疗效,值得临床进一步推广。

Objective：To discuss the efficacy and complications of transurethral resection of bladder tumor.Method：From January 2012 to February 2014,88 cases of bladder tumor in our hospital were selected as the research objects,they were divided into the laparotomy group and the electric cutting group according to the principle of random assignment,each group had 44 cases.The laparotomy group was given conventional open tumor resection and electric cutting group was given transurethral resection treatment,they were followed up for 1 year,the operation time,blood loss,length of stay,catheter indwelling time,complications and recurrence of 1,3,6 and 12 months after operation of two groups were compared.Result：The operation time,blood loss,length of stay and catheter indwelling time in the electric cutting group were lower than those of the laparotomy group,the differences were statistically significant（P〈0.05）.The complication rate of the electric cutting group was lower than that of the laparotomy group,the difference was statistically significant（P〈0.05）.Recurrence rate of 1,3,6 and 12 months after operation in the electric cutting group were lower than those of the laparotomy group,the differences were statistically significant（P〈0.05）.Conclusion：For the treatment of transurethral electric cutting can effectively shorten the operation time,reduce the trauma of bladder tumor patients,reduce the occurrence ofpostoperative complications,it is conducive to reduce the occurrence of recurrence and improve clinical curative effect,it is worth of further clinical promotion.
