摘要 目的探讨皮质醇(Co)、三碘甲状腺原氨酸(T3)、白细胞介素-1β(IL-1β)、胰岛素样生长因子-Ⅱ(IGF-Ⅱ)水平的变化在肺炎患儿营养状况评价中的价值。方法选取广州市某医院2015年1-12月收治的45例肺炎患儿作为肺炎组,另选取45例同期于医院进行健康体检的儿童作为对照组。分别采集2组儿童的静脉血,对Co,T3,IL-1β,IGF-Ⅱ水平进行检测并对结果进行分析。结果肺炎组患儿治疗前Co为(343.85±80.98)ng/mL,明显高于对照组儿童的(266.07±102.49)ng/mL;T3为(3.21±1.70)nmol/L,明显低于对照组儿童的(7.43±1.29)nmol/L,差异均有统计学意义(P<0.05)。肺炎组患儿治疗后T3为(5.77±0.74)nmol/L,明显高于治疗前的(3.21±1.70)nmol/L;Co水平为(189.43±109.01)ng/mL,明显低于治疗前的(343.85±80.98)ng/mL,差异均有统计学意义(P<0.05)。肺炎组患儿治疗前IL-1β为(0.34±0.13)ng/mL,明显高于对照组儿童的(0.20±0.04)ng/mL;而IGF-Ⅱ为(0.34±0.07)ng/mL,明显低于对照组儿童的(0.62±0.17)ng/mL,差异均有统计学意义(P<0.05)。肺炎组患儿治疗后IL-1β为(0.22±0.03)ng/mL,明显低于治疗前的(0.34±0.13)ng/mL;IGF-Ⅱ为(0.56±0.16)ng/mL,明显高于治疗前的(0.34±0.07)ng/mL,差异均有统计学意义(P<0.05)。结论肺炎患儿均存在不同程度的营养不良,可能与机体内激素合成以及细胞因子分泌紊乱有关。经过临床治疗,患儿体内激素以及细胞因子水平均有不同程度的改善,表明其营养状况得到了一定程度的提升。 Objective To investigate the value of cortisol (Co), triidothyronine (T3) , interleukin-1β (IL-Ⅱ), and insulin-like growth factor-Ⅱ (IGF-Ⅱ) changes in nutritional assessment of children with pneumonia. Methods Forty-five children with pneumonia treated at a Guangzhou hospital from January to December, 2015 were compared with another 45 healthy children who received health checkup at the same hospital during the same time period (control group). Blood samples were collected in both groups, and then the Co, T3, IL-1β, and IGF- Ⅱ levels were measured and compared. Results The pre-treatment level of Co in the pneumonia group was significantly higher than that in the control group (343.85±80.98) ng/mL vs. (266.07± 102.49) ng/mL, while the pre-treatment level of T3 was significantly lower compared with the control group (3.21±1.70) nmol/L vs. (7.43±1.29) nmol/L (both P〈0.05). In the pneumonia group, the post-treatment level of T3 was significantly higher than its pre-treatment level (5.77±0.74) nmoi/L vs. (3.21 ± 1.70) nmol/L, while the post-treatment level of Co was significantly lower than its pre-treatment level (189.43 ±109.01) ng/mL vs. (343.85±80.98) ng/mL (both P〈0.05). The pre-treatment level of IL-1β in the pneumonia group was signif-cantly higher than that in the control group (0.34±0.13) ng/mL vs. (0.20±0.04) ng/mL, whereas the pre- treatment level of IGF-H was significantly lower compared with the control group (0.3±0.07) ng/mL vs. (0.62±0.17) ng/mL (both P〈0.05). In the pneumonia group, the post-treatment level of IL-1β was signifi- cantly lower than its pre-treatment level (0.22±0.03) ng/mL vs. (0.34±0.13) ng/mL, while the post-treat- ment level of IGF-Ⅱ was significantly higher than its pre-treatment level (0.56±0.16) ng/mL vs. (0.3± 0.07) ng/mL (both P〈0.05). Conclusion Children with pneumonia often have varying degrees of malnutrition, which may be associated with disturbances in hormone synthesis and cytokine secretion. After proper clinical treatment, the hormone and cytokine levels are found to be improved, indicating an improved nutritional status of these children.
