摘要
目的探讨血清白介素-1(IL-1)与粒细胞集落刺激因子(G-CSF)在胎膜早破(PROM)及组织学绒毛膜羊膜炎(HCA)早期诊断中的临床意义。方法选择2011年7~12月在该院产科住院分娩的无症状足月PROM孕妇36例(研究组)与正常妊娠分娩孕妇50例(对照组)作为研究对象;采用免疫组化法检测产后胎膜破口边缘处胎膜组织,确定有无HCA。将胎膜组织病理检测有HCA的孕妇作为HCA组,无HCA的为非HCA组,通过酶联免疫吸附试验(ELISA)测定不同组别研究对象中血清IL-1及G-CSF的表达水平,并进行对比分析。结果 (1)研究组孕妇中HCA的发生率[63.89%(23/36)]高于对照组[4.00%(2/50)],差异有统计学意义(P〈0.05);(2)研究组孕妇血清IL-1及G-CSF水平明显高于对照组,差异均有统计学意义(P〈0.05);(3)HCA组孕妇血清IL-1及G-CSF水平明显高于非HCA组,差异均有统计学意义(P〈0.05)。结论血清IL-1及G-CSF水平升高在PROM合并HCA方面具有一定的临床意义,联合检测有利于早期预测及诊断。
Objective To investigate the clinical significance of serum interleukin-1(IL-1) and granulocyte colony-stimulating factor(G-CSF) in premature rupture of membrane(PROM) and histological chorioamnionitis(HCA). Methods A total of36 full term pregnant women with PROM and without symptoms in our hospital from July to December 2011 were selected as the research group and 50 cases of pregnant women with normal delivery were selected as the control group;the immunohistochemical method was adopted to simultaneously examine the embryonic membrane at the edge of fetal membrane rupture for determining HCA. The pregnant women with HCA by fetal membrane histopathological examination were taken as the HCA group and non-HCA as the non-HCA group. The levels of IL-1 and G-CSF were detected by enzyme linked immunosorbent assay(ELISA).Results(1)The HCA occurrence rate in the research group was 63.89%(23/36),which was higher than 4.00%(2/50) in the control group,the difference was statistically significant(P〈0.05);(2)the levels of serum IL-1 and G-CSF in the research group were higher than those in the control group with statistical difference(P〈0.05);(3)the levels of IL-1 and G-CSF in the HCA group were higher than those in the non-HCA group,the differences were statistically significant(P〈0.05). Conclusion The increase of IL-1 and G-CSF levels has certain clinical significance in the aspect of PROM complicating HCA,and their combined detection is conducive to early prediction and diagnosis.
