牙周基础治疗联合辅酶Q10注射液对牙周炎伴冠心病患者细胞因子的影响

Effect of periodontal basic treatment combined with coenzyme Q10 on cytokines in serum and gingival crevicular fluid of periodontitis with coronary heart disease

目的探讨牙周基础治疗联合辅酶Q10注射液对牙周炎伴冠心病患者细胞因子水平影响。方法选取确诊牙周炎合并冠心病患者68例,随机分成2组,每组34例。对照组予牙周基础治疗,观察组在此基础上予辅酶Q10注射液治疗,连续治疗2 w,检测患者血清及龈沟液中炎性因子水平,观察牙周指数及临床疗效。结果与治疗前比,血清及龈沟液中肿瘤坏死因子α(TNF-α)、白介素1β(IL-1β)、IL-6及IL-8水平降低(P<0.05),菌斑指数(plaque index,PLI)、龈沟出血指数(sulcus bleeding index,SBI)、牙周袋探诊深度(periodontal depth,PD)以及临床附着丧失(clinical attachment loss,CAL)均降低(P<0.05);与对照组比较,观察组血清及龈沟液中TNF-α、IL-1β、IL-6及IL-8水平较低(P<0.05),PLI、SBI及PD牙周指数较低(P<0.05),临床疗效较高(P<0.05)。结论辅酶Q10注射液可明显改善牙周炎伴冠心病患者牙周炎症,且降低血清及龈沟液中IL-6与TNF-α水平。

Objective To investigate the effect of periodontal basic treatment combined with coenzyme Q10 on cytokines in serum and gingival crevicular fluid of periodontitis with coronary heart disease. Methods A total of 68 patients with periodontitis with coronary heart disease were collected and randomly divided into 2 groups with 34 cases in each group. Patients in the control group were treated by periodontal basic treatment,patients in the experimental group were treated on the basis of the control group with coenzyme Q10 injection,continuous treatment for 2 weeks,the inflammatory factors levels in serum and gingival crevicular fluid were detected and the periodontal indexes and clinical efficacy were observed. Results Compared with before treatment,the TNF-α,IL-1β,IL-6 and IL-8 levels of serum and gingival crevicular fluid were decreased（ P〈0. 05）,the plaque index（ PLI）,sulcus bleeding index（ SBI）,periodontal depth（ PD） and clinical attachment loss（ CAL） were decreased（ P〈0. 05）; compared with the control group,the TNF-α,IL-1β,IL-6 and IL-8 levels in serum and gingival crevicular fluid of the experimental group were lower（ P〈0. 05）,PLI,SBI and PD periodontal indexes were lower（ P〈0. 05）,the clinical curative effect was higher（ P〈0. 05）. Conclusion Coenzyme Q10 injection in the treatment of patients of periodontitis with coronary heart disease could significantly improve the periodontal inflammation,and reduce the IL-6 and TNF-α levels in serum and gingival crevicular fluid.
