三袢两针单孔微型腹腔镜小儿腹股沟疝高位结扎术(附56例报告)

Single port mini-laparoscopic high ligation of hernia sac in pediatric inguinal hernia with three loops-two needles: with a report of 56 cases

目的：探讨三袢两针单孔微型腹腔镜技术治疗小儿腹股沟疝的疗效与优势。方法：为56例腹股沟疝患儿行三袢两针单孔微型腹腔镜手术,分析术中操作流程及术后疗效。结果：56例共64侧小儿腹股沟疝手术顺利完成,手术时间6-15 min,平均（9.5±1.8）min/侧。术后患儿均恢复顺利,无术后出血、阴囊积液、阴囊血肿等并发症发生,均于术后第2-3天出院。随访6-9个月,无复发病例。结论：三袢两针单孔微型腹腔镜小儿腹股沟疝高位结扎术具有操作简单、流程稳定、安全可靠、疗效良好的特点,既实现了结扎线的可选择性,又实现了双重结扎,值得临床推广。

Objective：To investigate a more scientific and maneuverable method of single port mini-laparoscopic high ligation of hernia sac in pediatric inguinal hernia with three loops-two needles. Methods： The intraoperative procedure and postoperative therapeutic effect of 56 children who underwent single port mini-laparoscopic high ligation of hernia sac in inguinal hernia with three loops-two needles were observed and analyzed. Results：Fifty-six children with 64 hernias were successfully operated. The operative time was 6-15 rain with the average of （9.5 ± 1.8 ） rain for each side. The postoperative recovery was uneventful, no complications occurred such as postoperative hemorrhage, scrotal hydrocele or scrotal hematoma. All patients were discharged from hospital on the second or third postoperative day. The 6-9 months postoperative follow-up showed a low incidence of complications and recurrence. Conclusions ： The single port mini-laparoscopic high ligation of hernia sac in pediatric inguinal hernia with three loops-two needles is safe, reliable and stable, has advantages of simple operation and good curative effect. This procedure provides choice of ligation suture and double ligations, is worthy of clinical popularization.
