HE Chengyao's Experience in Treating Painful Disease of Gynecology

何成瑶认为,妇科痛症的发病与解剖、生理密切相关。根据妇科痛症的发病特点,重视病变部位,强调从肝论治,提出治疗妇科痛症当以止痛为先,重在疏肝理气,肝郁得解,气血调和,通则不痛。临床辨证时可根据疼痛发生的时间、性质、部位,月经情况和包块的大小、部位以及体质和舌脉辨别寒热虚实,治疗时还须结合月经周期不同阶段,经前以调气祛瘀为主;经期以活血化瘀、理气止痛为主;经后以益气补肾、活血化瘀为主。

Prof. HE Chengyao thinks that painful disease of gynecology is closely related to anatomy and physiology. According to the characteristics of painful disease of gynecology,she attaches importance to the diseased region and emphasizes treatment from liver. And she puts forward that relieving pain should be regarded the first when treating painful disease of gynecology. She also emphasizes on soothing the liver to regulate Qi. The untied liver depression and the harmonious Qi and blood relieve pain. In clinical syndrome differentiation,she distinguishes cold from heat,and deficiency from excess according to the time,nature,location of the pain,the condition of menstruation,the size and location of enclosed mass,the habitus,the pulse and the tongue. She also treats with the menstrual cycle at different stages. She takes regulating Qi to removing stasis as the principal thing before menstruation. She takes promoting blood circulation to remove blood stasis and regulating Qi to alleviate pain as the principal thing during menstruation. She takes tonifying Qi and kidney and promoting blood circulation to remove blood stasis after menstruation.
