钼靶与B超对乳腺疾病的临床诊断价值探讨

Mammography and ultrasound the clinical diagnostic value of breast disease

目的对比钼靶影像质量控制后与B超检查在乳腺疾病诊断中的临床价值。方法收集2009年8月至2012年12月乳腺疾病女性患者共975例,并同时做了B超检查、钼靶影像质量控制后检查,均行穿刺/手术并经病理证实,分析B超和钼靶结果,比较两种方法单独与联合应用的准确性、敏感性、特异性。结果 410例乳腺癌患者中检出病灶数414个,565例乳腺良性病变患者中检出病灶数720个;其中,病理确诊为乳腺癌含2个癌性病灶数的有7例,病理确诊为乳腺良性病变含2个或2个以上良性病灶数的有70例。B超诊断乳腺疾病的敏感度为64.25%,而钼靶影像质量控制后诊断乳腺疾病的特异度为84.86%,两者联合应用准确度、敏感度、特异度均提高。结论探讨建立一套有效的检查方法,以提高乳腺疾病的诊断率。B超与X线钼靶检查各有优缺点,两者具有互补性,联合应用可提高乳腺疾病的诊断率,并可降低误诊率及漏诊率。

Objectives to contrast mammography image after quality control and clinical value of B ultrasonic examination in the diagnosis of breast disease. Methods To collect in August 2009 to December 2012,a total of 975 patients with mammary gland disease women,and at the same time do B to exceed check,mammography image quality control inspection,after piercing / operation and confirmed by pathology,analysis of the results of ultrasound and mammography,comparing two methods of single and combined application of accuracy,sensitivity and specificity. Results Comparing of 410 cases of patients with breast cancer detection was number 414,565 patients with breast benign lesions were detected in the lesions of 720; Among them,the pathological diagnosis of breast cancer,including the number of STD 2 carcinoma in 7 cases,the pathological diagnosis of breast benign lesions containing two or more the number of benign lesions in 70 patients. The sensitivity of ultrasound in the diagnosis of breast diseases was 64.25%,and mammography image quality control after the diagnosis of mammary gland disease 84.86%,both joint application accuracy,sensitivity,speciality degree were improved. Conclusions According to establish a set of effective inspection method is discussed,and in order to improve the diagnosis of breast disease. B ultrasonic and X-ray mammography check each have advantages and disadvantages,the two are complementary,a joint application can improve the diagnosis of mammary gland disease,and can decrease the rate of misdiagnosis and missed diagnosis.
