我院开展药学监护促进中风类中成药合理应用的研究

Promoting Effect of Pharmaceutical Care on Reasonable Application of Chinese Patent M edicines for Stroke in the Hospital

目的探讨中风类中成药合理应用的有效途径,以及药学监护对其是否有促进作用。方法选取医院开展药学监护前(2014年)中风类中成药处方566份(A组)及开展药学监护后(2015年)的同类处方547份(B组),比较不合理应用的处方数,并从选用药物、给药剂量、给药方式及药物配伍4个方面分析不合理用药情况,对比两组不良反应发生率、住院时间、住院费用等。结果中风类中成药注射剂中使用频率最高的为活血化瘀类,口服剂中使用频率最高的为活血通络类,与辨证情况相符。药学监护开展后,医院中风类中成药的不合理应用处方数、4种不合理应用情况、不良反应发生率均明显减少(P<0.05),且在患者住院时间相当的情况下,平均住院总费用下降(P<0.05)。结论开展药学监护可促进中风类中成药的合理应用,值得进一步完善。

Objective To explore the effective way of rational use of Chinese patent medicines for stroke,and to study on the effect of pharmaceutical care in reasonable application of Chinese patent medicines for stroke. Methods 566 pieces of stroke medicine prescrip- tion （group A） in 2014 （before the implementation of pharmaceutical care in the hospital） and 547 pieces of prescription（group B） in 2015 （after the implementation of pharmaceutical care in the hospital） were chosen and the pieces of irrational drug use in the two groups were compared. The irrational drug use was analyzed through the following 4 aspects： selection of drugs, dosage, administration methods, and drug compatibility. The incidence rate of adverse reactions, hospitalization time, hospitalization expenses in the two groups were also compared. Results The highest usage frequency of traditional Chinese medicine（TCM） injections for stroke was Huoxae Huayu type; the highest usage frequency of traditional Chinese medicine oral liquid for stroke was Huoxue Tongluo type, which were both suitable for the TCM syndrome. After the implementation of pharmaceutical care in the hospital, the number of unreasonable appli- cation of prescription ,4 kinds of unreasonable application, and the occurrence rate of the adverse reactions all significantly decreased （P 〈 0.05）;the average hospitalization cost also decreased during the same hospitalization time（ P 〈 0. 05）. Conclusion Pharmaceuti- cal care has played an active role in promoting the rational use of Chinese traditional patent medicine in the hospital,which is worthy of further development and improvement.
