探讨小切口双Endobutton钛板在肩锁关节脱位喙锁韧带修复重建中的应用价值

Application value exploration of small-incision dual Endobutton titanium plates on repair and reconstruction of coracoclavicular ligament in acromioclavicular dislocation

目的探讨小切口双Endobutton钛板在肩锁关节脱位喙锁韧带修复重建中的临床应用。方法2012年4月。2015年9月选取肩锁关节脱位共106例作为研究对象，随机分为观察组与对照组，观察组给予小切口双En-dobutton钛板在肩锁关节脱位喙锁韧带修复重建治疗，对照组给予切开复位锁骨钩钢板内固定治疗，术后定期随访，对比两组研究对象术后恢复情况。结果术后对患者随访平均时间8-24个月。观察组功能恢复优良率为94．34％，对照组功能恢复优良率为75．47％，差异有统计学意义（P〈0．05）。术前，两组研究对象VAS疼痛评分、喙锁间距、肩锁移位距离与对照组比较，差异无统计学意义（P〉0．05）；术后末次随访时，观察组VAS疼痛评分、喙锁间距、肩锁移位距离均优于对照组，差异有统计学意义（P〈0．05）。结论小切口双Endobutton钛板在肩锁关节脱位喙锁韧带修复重建中的应用是一种安全，有效的方法，临床效果良好。

Objective To explore the clinical application of small-incision dual Endobutton titanium plates on repair and reconstruction of coracociavicular ligament in acromioclavieular dislocation.Methods From April 2012 to September 2015,106 cases with acromioclavicular dislocation were selected as research objects.They were randomly divided into observation group and control group.In the observation group,repair and reconstruction of coracoclavieular ligament in acromioelavicular dislocation was realized by small incision with dual Endobutton titanium plates,while in the control group,clevicular hook plote was adopted.Follow-up visits after surgery at regular intervals were needed,and postopera- tive recovery in both groups was compared.Results The average follow-up visits after surgery was from 8 to 24 months. In the observation group,the excellent and good rate of functional recovery was 94.34%,and the rate in the control group was 75.47%,with a statistical difference （P〈0.05）.Before surgery,there was no great difference in VAS score,coracoelav- icular distance,and acromioclavieular translocation distance in the two groups （P〉0.05）.During the last time follow-up visit after surgery,the VAS seore,coracoclavicular distance,and acromioclavicular translocation distance in the observa- tion group were all superior to those in the control group with statistical differences （P〈0.05）.Conclusion Application of small-incision dual Endobutton titanium plates on repair and reconstruction of corac0elavicular ligament in acromio- clavicular dislocation is safe and effective approach can obtain a favorable clinical effect.
