基于JCI标准的晚期癌症患者临终关怀模式的实践与研究

Research and practice of end-of-life care for cancer patients based on JCI standards

目的为提高晚期癌症患者临终关怀的质量，基于JCI标准对患者临终关怀的要求，对晚期癌症患者实施临终关怀模式的研究。方法通过建立系统、完整、持续改进的临终关怀工作流程，制订并落实晚期癌症患者评估、临终关怀制度，并通过PDCA不断进行改进，对2010年1月～2015年12月900例住院患者实施临终关怀措施。结果900例临终关怀患者总体满意度达到99．5％。结论基于JCI标准的晚期癌症患者的临终关怀模式，能提高临终关怀的整体水平，提高患者的满意度，是目前较好的临终关怀模式，具有应用价值。

Objective To provide and improve the quality of end-of-life care for advanced cancer patients,the Joint Commission International Accreditation Standards （JCI standards） for hospitals was applied in the practice of palliative care for patients with advanced cancer.Methods A set of systemic,complete and continuously-improving end-of-life care processes were developed and a system for evaluating and providing end-of-life care for advanced cancer patients was formulated and carried out.Based on the processes and the system and the constant improvements made to them with PDCA method,end-of-life care was provided to 900 inpatients from January 2010 to December 2015.Results The result showed that the overall terminally ill patient satisfaction reached 99.5%.Conclusion The JCI standards-based end-of-life care for advanced cancer patients can improve the overall quality of care at the end of life and patient sat- isfaction.Therefore,it is a relatively good model of palliative care at the present time.
