综合护理模式在毕Ⅱ式胃大部切除术后胆管结石内镜下逆行胰胆管造影的效果

Analysis of comprehensive nursing care for post-Billroth Ⅱ gastrectomy patients with bile duct stones undergoing ERCP

目的对应用综合护理模式对接受毕Ⅱ式胃大部切除术后出现胆管结石的患者在进行内镜下逆行胰胆管造影（ERCP）期间实施护理的临床效果进行研究。方法选择我院2013年11月-2015年11月收治的毕Ⅱ式胃大部切除术后出现胆管结石的患者88例，按照随机数字表法将患者分为对照组和观察组，各44例。对照组患者采用常规手术护理模式在ERCP期间实施护理；观察组患者采用综合护理模式在ERCP期间实施护理。比较两组患者对毕Ⅱ式胃大部切除术后出现胆管结石行ERCP期间的护理服务的满意度、胆管结石住院治疗总时间、在行ERCP期间出现的不良反应人数。结果观察组患者对毕Ⅱ式胃大部切除术后出现胆管结石行ERCP影期间的护理服务的满意度（95．5％）明显高于对照组（77．3％）；胆管结石住院治疗总时间[（7．69±1．08）d]明显短于对照组[（11．50±2．86）d]；在ERCP期间出现的不良反应人数（1例）明显少于对照组（7例）。结论应用综合护理模式对接受毕Ⅱ式胃大部切除术后出现胆管结石的患者行ERCP期间实施护理的临床效果明显。

Objective To study the clinical effect of application of comprehensive nursing care for post-Billroth Ⅱ gas- trectomy patients with bile duct stones undergoing endoscopic retrograde cholangiopancreatography（ERCP）.Metbods 88 patients who suffered from bile duct stones after Billroth Ⅱ gastrectomy in our hospital from November 2013 to November 2015 were selected and divided into control group and observation group according to the random number table method,44 cases in each group.The routine surgical nursing mode was administered for patients in control group during ERCP;patients in observation group were nursed by using the comprehensive nursing mode during ERCP.The satisfaction rate,the total length of hospitalization for treatment of bile duct stones and the patients with adverse reac- tions of two groups were compared.Results For post-Billroth Ⅱgastrectomy patients with bile duct stones undergoing ERCP in observation group,satisfaction rate of the nursing service was 95.5%,higher than that was 77.3% of control group;the total length of hospitalization for treatment of bile duct stones in the hospital in observation group was （7.69± 1.08） d, significantly shorter than that was （11.50±2.86） d of control group;patients with adverse reactions during ERCP in observation group was 1 case,significantly less than that was 7 cases of control group.Conclusion The application of comprehensive nursing care for post-Billroth Ⅱ gastreetomy patients with bile duct stones undergoing ERCP has proved its obvious clinical effectiveness.
