护理干预预防剖宫产下肢深静脉血栓的价值评析

The evaluation of nursing intervention on prevention of deep venous thrombosis of lower limb in cesarean section

目的研究并探讨护理干预预防剖宫产下肢深静脉血栓的临床价值。方法选取2013年1月～2015年12月我院收治的剖宫产手术产妇中按照就诊顺序抽取100例产妇作为研究对象，采取计算机随机分组法，将其分为对照组与观察组．各50例。对照组实施常规护理干预，观察组实施综合护理干预。比较两组产妇剖宫产术后的下肢深静脉血栓发生率、大腿周径差值、小腿周径差值、VAS疼痛评分以及生活质量评分，并对两组产妇干预前后的焦虑评分进行比较。结果观察组产妇剖宫产术后的下肢深静脉血栓发生率为2％，明显低于对照组的14％（P〈0．05）。与对照组比较，观察组大腿周径差值、小腿周径差值、VAS疼痛评分均明显减小（P〈0．05），观察组的生活质量评分明显升高（P〈0．05）。与干预前比较，干预后两组产妇的焦虑评分均明显降低（P〈0．05），干预后，观察组的焦虑评分较对照组明显降低（P〈0．05）。结论在剖宫产产妇的围术期，实施综合护理干预可有效减少下肢深静脉血栓的发生．缓解下肢肿胀和疼痛情况。改善产妇的心理状况和生活质量。

Objective To study and discuss the clinical value of nursing intervention in prevention of deep venous thrombosis of lower limb in cesarean section.Methods In January 2013 to December 2015,according to the order of treatment,in this stage,100 cases of cesarean section maternal in our hospital were selected,100 cases of pregnant wom- en were divided into the control group and the observation group by taking computer randomization method,each group had 50 cases.The control group was given routine nursing intervention,and the observation group was given comprehen- sive nursing intervention.After cesarean section,the lower extremity deep vein thrombosis occurrence rate,thigh circum- ference difference,calf week diameter difference,VAS pain scores and the scores of quality of life were compared be- tween two groups,and before and after the intervention,the maternal anxiety scores of two groups were compared.Results The incidence of deep venous thrombosis after cesarean section in the observation group was 2%, which was signifi- cantly lower than that of control group which of 14% （P〈0.05）.Compared with the control group,the thigh difference, leg circumference,VAS pain score of the observation group were significantly smaller （P〈0.05）,the socre of life quality in the observation group was significantly higher （P〈0.05）.Compared with before intervention,the maternal anxiety scores of two groups were significantly lower after the intervention （P〈0.05）,but after the intervention,the anxiety score of the ob- servation group was significantly lower than that of control group （P〈0.05）.Conclusion The implementation of compre- hensive nursing intervention can effectively reduce the incidence of deep venous thrombosis of lower extremity,alleviate the swelling and pain of lower limbs,and improve the psychological status and life quality of the pregnant women.
