静脉用药调配中心9153帖退药中不合理用药医嘱分析

Analysis of irrational drug prescriptions in 9 153 drug repercussions in pharmacy intravenous admixture service

目的分析我院静脉用药调配中心退药医嘱中不合理用药的因素,促进临床合理用药。方法采用回顾性分析法,对我院静脉用药调配中心2014-01~2014-12间9 153帖退药中的不合理用药医嘱进行分析。结果不合理用药医嘱引起的退药为178帖,与其他原因引起的退药相比所占比例较低,仅为1.94%。不合理用药医嘱类型包括给药剂量不合理、给药频次不合理、溶媒的种类及用量选择不合理、药物间配伍禁忌等。结论对静脉用药调配中心不合理用药情况进行分析,可促进用药的合理性,提高静脉药物治疗的安全性和有效性。

Objective To analyze the factors of irrational drug use in 9 153 drug repercussions in pharmacy intravenous admixture service （PIVAS） for promoting rational clinical drug administration. Methods Irrational drug prescriptions in 9 153 drug repercussions in PIVAS of our hospital from January to December 2014 were analyzed retrospectively. Results Totally 178 drug repercussions were caused by irrational drug prescription, and the proportion was 1.94%,which was lower than that of other reasons. The reasons of irrational drug prescription included excessive dosage,incorrect medication interval, improper selection of solvent type and volume, incompatible drug use, etc. Conclusion It is helpful to promote drug rationality and improve security and effectiveness of drugs by analyzing the causes of irrational drug use in PIVAS.
