高血压合并代谢综合征患者接受缬沙坦联合二甲双胍治疗的降压效果及血清代谢指标评估

Assessment of antihypertensive efficacy of valsartan combined with metformin treatment and serum metabolic indexes of patients with hypertension and metabolic syndrome

目的：评估高血压合并代谢综合征患者接受缬沙坦联合二甲双胍治疗的降压效果及血清代谢指标水平变化。方法：收集2013年11月~2015年11月在本院接受住院治疗的高血压合并代谢综合征患者72例作为本次研究对象。按照随机数表法将患者分为观察组及对照组,各36例,对照组接受单独缬沙坦治疗,观察组接受缬沙坦联合二甲双胍治疗,对比两组治疗后血压、血生化指标及左心室舒张功能等差异。结果：观察组接受12周治疗后,收缩压（SBP）、舒张压（DBP）值及血清甘油三酯（TG）、低密度脂蛋白胆固醇（LDL-C）、胰岛素、胰岛素抵抗指数（IR）、抵抗素、瘦素、视黄醇结合蛋白4（RBP4）水平均低于对照组（P〈0.05）,血清高密度脂蛋白胆固醇（HDL-C）、脂联素（APN）、内脂素（Visfatin）及超声下左心室舒张早期运动速度峰值（EA）值高于对照组（P〈0.05）。结论：高血压合并代谢综合征患者接受缬沙坦联合二甲双胍治疗,在降压、优化血清指标及保护心功能等方面均有优势。

Objective： To evaluate the antihypertensive efficacy of valsartan combined with metformin treatment and serum metabolic indexes of patients with hypertension and metabolic syndrome. Methods： A total of 72 cases of patients with hypertension and metabolic syndrome who received inpatient treatment in our hospital from November 2013 to November 2015 were the research subjects. According to random number table,patients were divided into observation group（ n = 36） and control group（ n = 36）; control group received valsartan treatment alone and observation group received valsartan combined with metformin treatment. Differences in blood pressure,blood biochemical indexes,left ventricular diastolic function and so on were compared between two groups after treatment. Results： Systolic blood pressure（ SBP） and diastolic blood pressure（ DBP） values as well as serum triglyceride（ TG）,lowdensity lipoprotein（ LDL-C）,insulin,insulin resistance index（ IR）,resistin,leptin and retinol binding protein 4（ RBP4） levels of observation group after 12 weeks of treatment were significantly lower than those of control group（ P〈0. 05）,and serum high-density lipoprotein（ HDL-C）,adiponectin（ APN）,Visfatin and ultrasonic left ventricular early diastolic peak velocity（ EA） were significantly higher than those of control group（ P〈0. 05）. Conclusions： Valsartan combined with metformin therapy for patients with hypertension and metabolic syndrome has advantages in reducing blood pressure,optimizing serum indexes,protecting cardiac function and other aspects.
