纵隔异位甲状旁腺的外科治疗

Surgical treatment of ectopic mediastinal parathyroid

目的:分析纵隔异位甲状旁腺的手术疗效和病理特征。方法:回顾性分析1995年5月至2015年5月21例入住我科行手术治疗的纵隔异位甲状旁腺病人的临床资料。结果:男13例,女8例,平均年龄(56.5±8.5)岁。胸腔镜辅助手术(video associated thoracic surgery,VATS)切除9例,开胸手术13例,其中1例为VATS后2次开胸手术治疗。病理诊断为腺瘤16例(76.2%),腺瘤样增生4例(19.0%),腺癌1例(4.8%)。纵隔异位甲状旁腺直径为(2.2±1.2)cm。结论:纵隔异位甲状旁腺病理类型多为腺瘤,外科手术疗效好,尤其是VATS具有独特的临床优势。

Objective To investigate the effect of surgical treatment for ectopic mediastinal parathyroid and the pathological features. Methods The clinical data of 21 patients with ectopic mediastinal parathyroid who underwent the surgery in our department between May 1995 and May 2015 were retrospectively analyzed. Results There were 13 men and 8 women with a mean age of（56.5±8.5） years. Video associated thoracic surgery（VATS） was performed in 9 cases and open thoracic surgery in 13 cases including one case with secondary open surgery after VATS one month later. There were adenoma diagnosed pathologically in 16 cases（76.2%）, hyperplasia in 4 cases（19.0%） and adenocarcinoma in one case（4.8%）. The averaged size of ectopic mediastinal parathyroid was（2.2±1.2） cm. Conclusions Adenoma was the major type of ectopic mediastinal parathyroid. Surgical resection was demonstrated to be an effective therapy especially VATS with unique clinical advantages.
