乳腺囊内乳头状癌:49例临床特征及预后分析

Breast intracystic papillary carcinoma:clinical features and prognosis of 49 cases

目的 ：分析乳腺囊内乳头状癌的临床病理特征、预后及治疗。方法 ：收集49例乳腺囊内乳头状癌的临床资料和随访资料,分析其临床病理特征,并探讨不同临床病理特征对预后的影响。结果 ：49例乳腺囊内乳头状癌患者的中位年龄为64岁（范围：29~79岁）。中位随访时间为61个月（范围：24~84个月）。5年总生存率为75.0%,5年无病生存率为60.6%。保乳手术组12例患者的5年总生存率为76.9%,乳房全切组37例患者的5年总生存率为73.8%,2组的差异无统计学意义（P=0.935）。结论 ：乳腺囊内乳头状癌是一种非常少见的乳腺癌类型,预后较好。保乳手术和乳房全切术均可作为乳腺囊内乳头状癌的手术治疗方式。

Objective： To investigate the clinical and pathological characteristics of breast intracystic papillary carcinoma （IPC） and discuss its prognosis and treatment. Methods： The clinical and follow-up information of 49 patients with breast IPC was collected to analyze the clinicopathological characteristics and explore the influences on prognosis. Results： The median age of 49 patients with breast IPC was 64 years old （range： 29-79 years）. The median follow-up time was 61 months （range： 24-84 months）. The 5-year overall survival and disease-free survival rates were 75.0% and 60.6%, respectively. The 5-year overall survival rates of 12 patients receiving breast-conserving surgery and 37 patients receiving mastectomy were 76.9% and 73.8%, respectively （P = 0.935）.Conclusion： Breast IPC is a rare breast malignancy and has been perceived to have a good prognosis. Breast-conserving surgery and mastectomy have been utilized in the treatment of breast IPC.
