老年股骨粗隆间骨折通过股骨头置换术治疗的效果分析

目的探讨老年股骨粗隆间骨折通过股骨头置换术治疗的效果。方法研究该院2013年4月—2016年1月期间随机抽取的80例老年股骨粗隆间骨折患者,分为对照组与观察组各40例,其中对照组运用DHS内固定术,观察组运用人工股骨头置换术,分析两组患者治疗效果差异。结果在手术时长上,观察组(90.1±12.6)min与对照组(93.7±13.5)min差异无统计学意义,P>0.05;在手术出血量上,观察组(376.2±71.1)m L与对照组(369.7±73.6)m L差异无统计学意义,P>0.05;在住院时长上,观察组(17.3±6.2)d显著短于对照组(27.9±5.7)d,P<0.05;在负重训练起始时间上,观察组(19.2±3.2)d显著短于对照组(48.1±12.3)d,P<0.05;在术后并发症率上,观察组为2.5%,对照组为22.5%,组间差异有统计学意义,P<0.05;在HSS良好率上,观察组为82.5%,对照组为57.5%,P<0.05,差异均有统计学意义。结论老年股骨粗隆间骨折通过股骨头置换术治疗可以有效的提升恢复速度,提供恢复效果。

Objective To investigate elderly intertrochanteric fracture of the femoral head by replacement therapy. Methods Research in our hospital in April 2013 to January 2016 randomly selected 80 cases of elderly femoral coarse intertrochanteric fracture patients, divided into control group and observation group, 40 cases in each group. The control group were treated with DHS internal fixation, and the observation group with artificial femoral head replacement and analysis of two groups of patients treatment effect difference. Results HSS operative surgery in length, the observation group（90.1 ±12.6 min） and control group（93.7 ± 13.5 min） had no significant difference, P〉0.05. The amount of bleeding, observation group（376.2 ± 71.1） ml and control group（369.7± 73.6） ml had no significant difference, P〉0.05. In the hospital, the observation group（17.3 ± 6.2 d） was significantly shorter than that of the control group（27.9 ± 5.7） d, P〈0.05. In weight training starting time, the observation group（19.2 ±3.2 d） was significantly shorter than that of the control group 48.1± 12.3）d, P〈0.05. The postoperative complication rate, the observation group was 2.5%, while the control group was 22.5%, group difference significantly, P〈0.05. The rate of excellent and good, the observation group was 82.5%, while the control group was 57.5%, P〈0.05. The difference was statistically significant. Conclusion The elderly intertrochanteric fracture through the femoral head replacement can effectively enhance the recovery effect.
