生物型半髋置换术治疗老年股骨颈骨折

Treatment of femoral neck fracture in elderly patients with uncemented semi hip replacement

目的评价生物型半髋置换术治疗老年股骨颈骨折的临床效果。方法采用生物型半髋置换术治疗86例老年股骨颈骨折患者。结果手术时间40-60 min,术中出血量100-200 ml。86例均获得随访,时间3个月-4年。患者术后7-10 d扶双拐下地,2.5-3个月完全负重行走,髋关节功能接近伤前水平,生活恢复自理。术后1例早期行走痛,2例轻度跛行,1例假体周围骨折。髋关节功能Harris评分：优80例,良4例,可2例。结论生物型半髋置换术治疗老年股骨颈骨折具有手术快、出血少、风险小以及早期临床效果满意等优点。

Objective To evaluate the clinical effect of the treatment of femoral neck fracture in elderly patients with uncemented semi hip replacement. Methods Eighty-six cases of elderly patients with femoral neck fracture were treated with uncemented semi hip replacement. Results The operation time was 40 - 60 min,and the volume of intraoprative bleeding was 100 - 200 ml. Eighty-six cases were followed up from 3 months to 4 years. Patients helped crutches under after 7 - 10 days,full weight-bearing walked in 2. 5 - 3 months postoperation,and hip function was close to before injury level,recovered life self-care. 1 case was early postoperative walking pain,2 patients had mild claudication,1 case was fracture around the prosthesis. Harris score results of hip joint function was excellent in 80 cases,good in 4,fair in 2. Conclusions The treatment of femoral neck fracture in elderly patients with uncemented semi hip replacement has advantages of quick operation,less bleeding,less risk,and satisfactory clinical results.
