湿性疗法联合生长因子治疗Ⅲ级及以上放射性皮肤损伤的临床观察

摘要 目的探讨湿性疗法联合生长因子治疗Ⅲ级及以上放射性皮肤损伤的临床效果。方法将2012年5月至2015年5月在我院行放射治疗发生Ⅲ级及以上放射性皮肤损伤的86例患者使用随机数字表法随机分为观察组和对照组。观察组采用湿性疗法联合生长因子治疗,对照组采用传统方法治疗,比较两组患者治疗结局、治愈时间及治疗费用。结果观察组和对照组Ⅲ级放射性皮肤损伤均完全治愈,观察组Ⅳ级治愈率为92.9%,对照组Ⅳ级治愈率为25%,差异有统计学意义(P<0.05)。观察组Ⅲ级放射性皮肤损伤治愈所需时间为(18.3±3.6)d,对照组为(33.3±6.0)d;观察组Ⅳ级放射性皮肤损伤治愈所需时间为(39.5±5.3)d,对照组为(53.0±3.6)d,观察组较对照组治愈时间明显缩短,差异有统计学意义(P<0.05)。观察组中Ⅲ级放射性皮肤损伤平均治愈费用为(161.7±16.1)元,对照组为(155.6±17.5)元;观察组中Ⅳ级平均治愈费用为(284.5±24.1)元,对照组为(313.0±23.4)元,两组治愈费用差异无统计学意义(P>0.05)。结论湿性疗法联合生长因子治疗能够有效缩短Ⅲ级及以上放射性皮肤损伤的治愈时间,而不增加患者的治疗费用,尤其适合Ⅳ级放射性皮肤损伤的治疗,是治疗Ⅲ级及以上放射性皮肤损伤的较理想的方法之一,值得临床推广应用。 Objective To investigate clinical efficacy of moist therapy combined with growth factor in patients with degreeⅢ or above radiation-induced skin injury. Methods 86 patients received radiotherapy and developed degree Ⅲ or above radiation-induced skin injury in the hospital from May 2012 to May 2015 were randomized into observation group and control group using a random number table. Patients in the observation group received growth factor combined moist therapy,while patients in the control group received traditional treatment. Treatment outcomes,healing time and treatment costs were compared. Results Patients with degreeⅢskin injury in observation group and control group were all completely cured. And the cure rate of degreeⅣinjury were 92. 9% in the observation group and 25% in control group. The difference was statistically significant(P〈0. 05). The healing time for degreeⅢskin injuery were(18. 3 ± 3. 6)d in observation group and(33. 3 ± 6. 0)d in control group. And the healing time for degree Ⅳ injury were(39. 5 ± 5. 3) d and(53. 0 ± 3. 6) d respectively in observation and control group. The healing time was significantly shortened in observation group(P〈0. 05). In observation group,to cure a patient with degreeⅢskin lesion needed a cost of(161. 7 ± 16. 1)yuan. And in control group,the average healing cost was(155. 6 ± 17. 5) yuan. And the average healing cost were(284. 5 ± 24. 1) yuan and(313. 0 ± 23. 4) yuan respectively in observation and control group. But the difference didn’ t demonstrated statisticsignificance( P〉0. 05 ) . Conclusion Moist therapy combined with growth factor effectively short-ened the healing time of degreeⅢor above radiation-induced skin injury without raising the cost of treatment. The therapy was ideal for degree Ⅲ or above radiation-induced skin injury,especially for the degree Ⅳ skin injury.
