摘要
目的探究特发性血小板减少性紫癜(ITP)病患血清中可溶性Fas(s Fas)、可溶性Fas配体(s Fas L)、β2-微球蛋白(β2-MG)以及细胞因子的水平,并分析其相关性。方法选择2013年1月至2014年12月在海南省澄迈县人民医院接受治疗的ITP病患73例,并记为观察组。再随机选择同期在本院进行健康体检的志愿者73名,同时记为对照组。对比两组血清中s Fas、s Fas L、β2-MG及相关细胞因子水平,最后进行相关性分析。结果观察组患者s Fas、s Fas L、β2-MG、CD4+及CD28+水平分别为(14.94±2.08)、(0.67±0.13)、(2.63±0.45)、(25.61±4.28)及(32.74±7.03),显著高于对照组患者的(5.82±1.46)、(0.33±0.06)、(1.27±0.19)、(36.09±5.12)、(50.17±10.36),差异有统计学意义(P均<0.05);而观察组IL-2水平(0.31±0.07)低于对照组(0.62±0.18),差异有统计学意义(P<0.05)。相关性分析结果显示,s Fas、s Fas L及β2-MG与ITP呈正相关,而IL-2、CD4+及CD28+与ITP呈负相关,差异有统计学意义(P<0.05)。结论 s Fas、s Fas L和β2-MG参与了ITP的发病过程,可以应用于病变的诊断及分型。并且ITP病患免疫功能出现缺陷,对预后效果有一定的指导价值。
Objective To explore the correlation of idiopathic thrombocytopenic purpura(ITP) with serum levels of s Fas, s Fas L, beta 2- microglobulin(β2-MG) and cytokine. Methods 73 cases of ITP patients were selected from January 2013 to December 2014 in our hospital as observation group; 73 cases of healthy volunteers were randomly selected in our hospital as control group. Compared the two groups of serum levels of s Fas,s Fas L,2-MG and related cytokines. Results Thes Fas, sFasL,β2-MG,CD4+and CD28+levels of the observation group were 14.94 ±2.08,0.67 ±0.13, 2.63±0.45,25.61±4.28 and 32.74±7.03,respectively,which were higher than the control group(5.82±1.46, 0.33±0.06,1.27 ±0.19,36.09 ±5.12,and 50.17 ±10.36). The IL-2 level of the observation group was 0.31 ±0.07, which was lower than that of the control group(0.62 ±0.18).Correlation analysis showed that s Fas, s Fas L and 2-MG were positively correlated with ITP. IL-2 and CD4+were negatively correlated with ITP and CD28+; the differences were statistically significant(P 〈0.05). Conclusion s Fas, s Fas L and 2-MG were involved in the pathogenesis of ITP and could be used in disease diagnosis and typing. And the defect of immune function of the ITP patient has certain guiding value for the prognosis.
