鼻渊通窍颗粒对儿童慢性鼻窦炎鼻黏膜纤毛传输功能及炎性细胞因子的影响 被引量:20

摘要 目的观察鼻渊通窍颗粒治疗儿童慢性鼻窦炎的临床疗效,并探讨其对鼻黏膜纤毛传输功能和炎性细胞因子的影响。方法慢性鼻窦炎患儿276例随机分为对照组和观察组,各138例。对照组给予阿莫西林克拉维酸钾干混悬剂和氨溴索颗粒治疗,观察组在对照组基础上加服鼻渊通窍颗粒。对比2组临床疗效、鼻黏膜纤毛输送率(MTR)、鼻黏膜纤毛清除率(MCC),以及治疗前后血清白细胞介素-2(IL-2)、IL-6、IL-10、干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)、TNF-β和C反应蛋白(CRP)表达水平差异。结果观察组总有效率90.58%,显著优于对照组的81.88%(P<0.05);治疗后3和7 d,观察组MTR和MCC显著高于对照组(P<0.01);2组IL-6和IFN-γ水平较治疗前上升,IL-2、IL-10、TNF-α、TNF-β和CRP较治疗前下降(P<0.05或P<0.01),除TNF-β,观察组上述炎症因子水平改善程度显著优于对照组(P<0.05或P<0.01)。结论鼻渊通窍颗粒能通过改善慢性鼻窦炎患儿的鼻黏膜纤毛功能及减轻炎症状态,改善其临床症状。 Objective To observe the clinical efficacy of Biyuan tongqiao granule in treatment of children with chronic sinusitis, and to investigate the influence on nasal mucociliary transport function and level of inflammatory factors. Methods A total of 276 children with chronic sinusitis were randomly divided into the control group and the observation group, with 138 cases in each group. The amoxicillin and clavulanate potassium for suspension and ambroxol hydrochloride granules were administrated in both of two groups, while Biyuan tongqiao granule was additionally given in the observation group. The clinical efficacy, nasal mucociliary transport rate (MTR), nasal mucociliary clearance rate ( MCC), as well as levels of related inflammatory factors including Interleukin-2 ( IL- 2), IL-6, IL-10, interferon-γ(IFN-γ), tumor necrosis factor-α(TNF-α), TNF-β and C-reactive protein( CRP), were compared between two groups. Results The overall efficacy rate was 90.58% in the observation group and was 81.88% in the control group, with a significant difference (P 〈 0.05). the MTR and MCC were significant higher in the observation group than those in the control group 3 day and 7 days after treatment (P 〈 0.01 ). Levels of IL-6 and IFN-γ were increased, and levels of IL-2, IL-10, TNF-α, TNF-β and CRP were decreased in both of two groups after treatment ( P 〈 0.05 or P 〈 0.01 ). The observation group received better improvement in all inflammatory factors levels except for TNF-α compared with the control group ( P 〈 0.05 or P 〈 0.01 ). Conclusion Biyuan tongqiao granule can enhance the clinical efficacy by improving the nasal mucociliary transport function and relieving inflammation.
