摘要
目的:利用服务设计的方法策略对老年慢性病患者延续性护理模式进行设计。方法:以服务设计的方法来对老年慢性病护理进行重新思考,以服务设计的基本原理为基础,从设计的层面对我国老年慢性病的延续性护理现存模式的壁垒进行归类分析。结果:通过对现在我国延续性护理现状的调查分析,得出其中存在的局限性,如:信息交接和护理服务的脱节;在社区、家庭中的辅助、护理设备设计缺失等。结论:延续性护理是我国慢性病治疗康复方面的一个重要发展方面,从服务设计的角度对其局限性进行重新设计能够很好地解决现实问题,提高老年人出院后的生活质量。
Objective： Using the ways of service design to design the model about continuous nursing of the Elderly Chronic diseases. Methods： Rethinks the continuous nursing of the Elderly Chronic diseases by using the ways of service design, based on the principles of service design, and classifies and analyzes the barrier about the continuity care of the Elderly Chronic diseases from the design level. Results： It is concluded that there are limitations （1 .The disconnect of Information transfer and Care services 2.Lack of auxiliary and care equipment which suitable use at community and home） to the continuous nursing. Through the investigation and analysis about the continuous nursing in our country, continuous nursing can be improved by using the Service design. Conclusion： Continuous nursing is an important treatment for chronic diseases. And from the perspective of service design, rethink its limits can be very good to solve practical problems and improve the life quality of old people after discharge.
