感染性休克患者下腔静脉管径及塌陷指数与PiCCO血流动力学指标的相关性 被引量:12

摘要 目的探讨感染性休克患者下腔静脉管径及塌陷指数与脉搏指示连续心排出量血流动力学指标的关系。方法对入住我院ICU符合条件的感染性休克36例患者进行液体复苏,于液体复苏后6 h记录全心舒张末容积(GEDV)、胸内血容量(ITBV)、每搏变异度(SVV)及心排指数(CI);同时记录肝后下腔静脉最大径(IVCmax)、最小径(IVCmin)及塌陷指数(IVCci)。结果 IVCmax与ITBV、CI为正相关性,而IVCmax与SVV为负相关性。IVCmin与GEDV、ITBV、CI为正相关性,IVCmin与SVV为负相关性。IVCci与ITBV、CI具有显著的负相关性。结论下腔静脉管径及塌陷指数与Pi CCO血流动力学指标具有相关性。 Objective To explore the correlation between inferior vena cava (IVC) , collaps- ible indexes and PiCCO hemodynamic parameters in patients with septic shock. Methods A total of 36 septic shock patients in ICU were treated with fluid resuscitation. Six hours after fluid resuscitation, global end diastolic volume (GEDV), intrathoracic blood volume (ITBV), stroke volume veriation (SVV) and cardiac index (CI) were recorded, and meanwhile the maximum radius variation of inferi- or vena cava( IVC ), the minimum radius variation of inferior vena cava ( IVCmin ) and collapsible index of in^rior vena eava (IVCei) were also recorded. Results IVCmax was positively correlated with lTBV and CI, while IVCmax was negatively correlated with SVV. IVCmax. was positively correlated with GEDV, ITBV and CI, and IVCmax was negatively correlated with SVV. IVCci had a significant negative correlation with CI, ITBV. Conclusion There are correlations between IVC, collapsible indexes and PiCCO hemodynamic parameters in patients with septic shock.
