保留神经宫颈癌根治术后膀胱功能的恢复及临床护理对策

Recovery of bladder function after radical hysterectomy with nerve preservation and clinical nursing countermeasures

目的探析保留神经宫颈癌根治术后膀胱功能的恢复及临床护理对策。方法选取120例接受保留神经宫颈癌根治术患者,按照就诊时间顺序分为常规护理组与综合护理组,各60例,对比2组手术时间、术中出血量、保留尿管天数、残余尿量、肛门排气时间、肛门排便时间、膀胱功能恢复及性功能评分。结果与常规护理组相比,综合护理组保留尿管天数缩短、术后第10日残余尿量减少、肛门排气时间与肛门排便时间缩短,差异有统计学意义(P<0.05)。综合护理组膀胱功能Ⅰ级患者所占比例高于常规护理组、Ⅲ级患者所占比例低于常规护理组,差异有统计学意义(P<0.05)。综合护理组性欲望、性唤起、阴道润滑度、性高潮、性生活满意度及性交疼痛评分均高于常规护理组,差异有统计学意义(P<0.05)。结论将综合护理应用于保留神经宫颈癌根治术中临床效果显著。

Objective To explore the recovery of bladder function after radical hysterectomy with nerve preservation and clinical nursing countermeasures. Methods A total of 120 radical hyster- ectomy patients with nerve preservation were divided into routine nursing group and comprehensive nursing group, 60 cases in each group. Operation time, intra-operative blood loss, urethra catheter indwelling time, residual urine volume, anal exhaust time, anus bowel time, bladder function recov- ery and sexual function were compared between two groups. Results There were significant differ- ences in urethra catheter indwelling time, residual urine volume 10 days after operation, the anus bowel and anus exhaust time between two groups （ P 〈 0.05 ）. In comprehensive nursing group, ratio of patients with level I of bladder function was significantly higher than routine nursing group, and ra- tio of patients with level III of bladder function was significantly lower than routine nursing group （P 〈 0.05）. In comprehensive nursing group, the scores of sexual desire, arousal, vaginal lubrica- tion degree, orgasm, sexual satisfaction degree and sexual intercourse pain were significantly higher than those in routine nursing group （P 〈 0.05 ）. Conclusion Comprehensive nursing is effective in treatment of radical hysterectomy patients with nerve preservation.
