中医药治疗白细胞减少症的思路与方法

The Thinking and Method of TCM in Treating Leucopenia

白细胞减少症的发病是由先天禀赋不足,体质虚弱;后天失于调理,耗伤气血所致。疾病乏源于脾,病本于肾;肾与脾的虚损是本病发病之关键。病久不愈可出现毒热入血、热毒败血等病机变化;虚、瘀、热是其主要病理表现;阴、阳、气、血亏损是其主要的病机演变特点。临床辨证为气血两虚、肝肾阴虚、脾肾阳虚、正虚血瘀4型,分别采用自拟升白系列方进行治疗。同时应注意辨证论治,审证求因;调补脾肾,以治其本;气血互生,阴阳互根;活血化瘀,贯穿始终;擅用诸参,活用人参;顾护胃气,健脾调中。

The causes of leucopenia were physical weakness due to congenital deficiency; Qi and blood de- pletion due to loss of postnatal regulation. The disease was originated in the spleen and rooted in the kidney; renal and spleen deficiency was the key point of leucopenia. Long-term disease could manifest the changes of the patho- genesis： toxin-heat invading the blood, transforming into sepsis; deficiency, stagnation and heat were its main patho- logical manifestations; the deficiency of Yin, Yang, Qi and blood were the features of main pathogenesis develop- ment; the disease could be differentiated as four patterns of dual deficiency of Qi-blood, liver-kidney Yin deficiency, spleen kidney Yang deficiency, healthy Qi deficiency and blood stasis, which were treated with prescription series of ShengBai respectively. Simultaneously, we should pay attention to six points： syndrome differentiation and treat- ment, signs to identify etiology; regulating and tonifying spleen and kidney in order to treat the root cause; mutual generation of Qi and blood, interdependence of Yin and Yang; promoting blood circulation and eliminating the stasis throughout; use all kinds of the ginseng actively; taking care stomach Qi, invigorating spleen and regulating the mid- dle energizer.
