冷冻球囊导管消融术治疗心房颤动12例临床分析

Analysis of 12 Cases of Atrial Fibrillation Treated by Cryoballoon Catheter Ablation

目的心房颤动(AF)是最常见的心律失常,冷冻球囊导管消融术(CBCA)操作简单、肺静脉狭窄及血栓栓塞等并发症发生率低,是AF有效治疗方法。本文总结CBCA治疗AF临床经验。方法回顾性分析2014年4—10月南昌大学第二附属医院经CBCA治疗的AF患者12例临床资料并文献复习。结果术中1例患者肺静脉隔离后AF终止,1例患者出现右侧膈神经麻痹。围术期患者均无脑卒中或短暂性脑缺血发作、腹股沟并发症以及心脏压塞、肺静脉狭窄等。12例患者3、6个月动态心电图复查无阵发性AF发生,2例心悸发作时心电图提示窦性心动过速。结论CBCA治疗AF安全有效,但其长期效果及预后需大样本、长时间随访进一步证实。

Objective Atrial fibrillation（ AF） is one of most common cardiac arrhythmias. Cryoballoon catheter ablation（ CBCA） is easy to be operated and has lower rate of complications,such as pulmonary vein stenosis and thrombo-embolism. We summarized clinical experiences of CBCA in this paper. Methods Clinical data of 12 patients with AF who were treated by CBCA in the Second Affiliated Hospital of Nanchang University from April to October 2014,and relevant literatures were reviewed. Results AF disappeared after pulmonary vein isolation for one patient. One patient experienced right diaphragmatic nerve palsy. None patient suffered from stroke, transient cerebral ischemia, complications in groin, cardiac tamponade and pulmonary vein stenosis during perioperative period. After procedure 3 and 6 months,and paroxysmal AF were not found by dynamic electrocardiogram for all patients. Two patients showed sinus tachycardia on electrocardiogram when palpitation occurred. Conclusion Cryoballoon catheter ablation is effective in treating atrial fibrillation. Long term effect and prognosis need to be observed with long term follow- up.
