健康者低剂量多层螺旋CT呼吸双相段支气管管径的定量研究 被引量:6

Quantitative Study on Segmental Bronchial Diameter at Inspiratory and Expiratory Phases by Low-dose MSCT in Healthy Subjects

摘要 目的定量研究健康者低剂量多层螺旋CT(MSCT)呼吸双相段支气管管径,为支气管镜检查及介入治疗、支气管疾病诊断提供参考数据。方法选取2015年3—6月在河北医科大学第一医院体检中心行健康体检者68例,其中男42例,女26例。采用低剂量MSCT扫描受试者全肺的呼吸双相,并将MSCT数据导入FUJIFILM Corporation SYNAPSE 3D软件进行后处理,选取双肺下叶段支气管走行平直段进行测量。观察所有受试者低剂量MSCT扫描结果,并比较不同性别受试者呼吸双相双肺下叶段支气管管径的长径(Din-L)及短径(Din-S)。结果 68例受试者低剂量MSCT扫描共显示双肺下叶段(右肺下叶背段、前基底段、内基底段、后基底段、外基底段及左肺下叶背段、前内基底段、外基底段及后基底段)支气管612支,其中男性378支,女性234支。男性受试者呼吸双相双肺下叶段支气管Din-L和Din-S均大于女性(P<0.05);男性与女性吸气相双肺下叶段支气管Din-L、Din-S均大于呼气相(P<0.05)。结论低剂量MSCT可以准确扫描出呼吸双相双肺下叶段支气管,且吸气相双肺下叶段支气管管径大于呼气相,男性呼吸双相支气管径大于女性。 Objective To quantitatively study the segmental bronchial diameter at inspiratory and expiratory phases by low - dose MSCT in healthy subjects and provide reference data for bronchoscopy and interventional therapy as well as diagnosis of bronchial diseases. Methods 68 healthy subjects in the Physical Examination Center of the First Hospital of Hebei Medical University were enrolled from March to June 2015,including 42 males and 26 females. The inspiratory and expiratory phases of the whole lung of subjects were scanned by low - dose MSCT. The MSCT data were imported into the FUJIFILM Corporation SYNAPSE 3D software for postprocessing. The measurement was conducted by selecting the weathervanes in the segmental bronchi of lower lobes of both lungs. The results of low - dose MSCT scanning of all subjects were observed. The long diameter(Din - L) and short diameter(Din - S)of the segmental bronchi of lower lobes of both lungs at inspiratory and expiratory phases of subjects with different genders were compared. Results Low - dose MSCT scanning for 68 cases showed 612 bronchi of lower lobes of both lungs(dorsal segment,anterior basal segment,medial basal segment,posterior basal segment and lateral basal segment of the lower lobe of right lung as well as dorsal segment,anterior basal segment,medial basal segment,lateral basal segment and posterior basal segment of the lower lobe of left lung),including 378 bronchi of in males and 234 bronchi in females. Din - L and Din - S of the lumens of the segmental bronchi of lower lobes of both lungs at inspiratory and expiratory phases in males were greater than those in females(P ﹤ 0. 05). Din - L and Din - S of the lumens of the segmental bronchi of lower lobes of both lungs at inspiratory phase in males and females were greater than those at expiratory phase( P ﹤ 0. 05) . Conclusion The segmental bronchi of lower lobes of both lungs at double phases can be accurately scanned by low - dose MSCT. Moreover,the bronchial diameter of lower lobes of both lungs at inspiratory phase is larger than that at expiratory phase and the bronchial diameter at inspiratory and expiratory phases in males is larger than that in females.
