颅脑损伤后血清中MMP-9、S-100B蛋白的表达对颅脑损伤预后及脑水肿的影响 被引量:8

摘要 目的 探讨颅脑损伤(TBI)患者血清中S-100B和基质金属蛋白酶-9(MMP-9)蛋白水平的变化,并分析其对TBI严重程度、预后及脑水肿的影响.方法 入选120例TBI患者,根据入院时格拉斯哥昏迷分级(GCS)进行分组、并选择30例健康体检者作为正常对照,采用酶联免疫吸附法(ELISA)测定伤后1、3、5、7d血中S-100B和MMP-9蛋白浓度的变化情况.分析血清S-100B和MMP-9蛋白浓度水平,及其与TBI入院时GCS评分、患者伤后3个月时格拉斯哥预后评分(GOS评分)及头颅CT提示脑水肿变化的相关性.结果 TBI组血清中S-100B和MMP-9蛋白浓度较对照组明显升高(P<0.05),同一时间内比较,重度TBI组血清中S-100B和MMP-9蛋白水平较轻度TBI组明显升高(P<0.05);S-100B和MMP-9蛋白水平与TBI患者严重程度和脑水肿体积呈正相关,与GCS和GOS评分呈负相关.TBI患者恢复不良组24例,血清S-100B蛋白浓度(1.353 ±0.0534) ng/ml,MMP-9蛋白浓度(2.032±0.0463) ng/ml;恢复良好组96例,血清S-100B蛋白浓度(0.495 ±0.0521)ng/ml,MMP-9蛋白浓度(0.963 ±0.0354) ng/ml,恢复不良组与恢复良好组之间血清S-100B和MMP-9蛋白浓度比较差异具有统计学意义(P<0.05).结论 血清中S-100B和MMP-9蛋白测定可作为判断TBI患者严重程度、预后及脑水肿的重要指标,为TBI救治提供途径.
