摘要
目的探讨实时组织弹性成像对肝硬化结节的诊断价值。方法对48例肝硬化患者的55个结节(直径≤2cm)进行实时弹性成像,采用应变率比值法定量测定肝硬化结节与周围肝组织的硬度比值。通过受试者工作特征曲线分析结节的良恶性特征。结果通过分析55个病灶的特征,确定应变率比值等于3.18为肝脏良恶性肿瘤的最佳诊断临界值,其诊断敏感度为88.24%,特异度为80.95%,准确性为83.64%,阳性预测值88.20%,阴性预测值81.00%。结论实时超声弹性成像应变率比值法是评价直径≤2cm的肝硬化结节良恶性的一种有效方法。
Objective To explore the value of real-time ultrasound elastography in the diagnosis of cirrhosis nodules of the live. Methods The real-time ultrasonic elastography was performed in 48 patients(55 nodules, the diameter 42 cm) with liver cirrhosis. The hardness ratio of nodules to liver tissues was calculated by the strain rate ratio method. The characteristics of benign and malignant of cirrhotic nodules was analyzed by the receiver operating characteristic (ROC) curve. Results By analyzing the characteristics of 55 cirrhotic nodules, the cutoff value of strain ratio of 3.18 was determined and the sensitivity was 88. 24%, specificity was 80. 95%, accuracy was 83.64%, positive predictive value was 88.20%, and negative predictive value was 81.00% for the diagnosis of benign and malignant liver tumors. Conclusion The real-time ultrasonic elastography strain ratio method is effective for the diagnosis of benign and malignant liver tumors 42 cm in diameter.
