术前抑癌消瘤汤联合新辅助化疗对结直肠癌术后淋巴结检出数目及癌组织中CD44v6和p27表达的影响

Effects of Preoperative Tumor- suppressor Decoction Combined with Neoadjuvant Chemotherapy on the Number of Lymph Nodes Detected and the Expression of CD44v6 and p27 in Cancer Tissues after Operation for Colorectal Cancer

目的：探究抑癌消瘤汤联合新辅助化疗术前治疗对结直肠癌患者根治术后淋巴结检出数目及癌组织中CD44v6、p27表达的影响。方法：回顾性分析2008年4月~2011年7月间入院就诊的所有确诊结直肠癌患者临床资料,从术前行新辅助化疗方案及联合抑癌消瘤汤口服方案的患者中各抽取49例,分别纳入联合组和对照组。对比两组患者5年存活率及生活质量差异,记录其根治术后淋巴结检出数目及肿瘤组织中CD44v6、p27表达情况。结果：联合组术后复查时淋巴结检出数目及KPS评分均显著高于对照组（P〈0.05]。治疗后,联合组肿瘤组织中CD44v6蛋白阳性表达率显著低于对照组（P〈0.05）,p27蛋白阳性表达率则显著高于对照组（P〈0.05）。联合组术后5年存活率显著高于对照组（P〈0.05）。结论：将抑癌消瘤汤联合新辅助化疗方案应用于结直肠癌的术前治疗中,可有效提升手术效果、促进病情转归,于患者预后提升有利。

Objective： To explore the effects of Preoperative Tumor-suppressor Decoction combined with neoadjuvant chemotherapy on the number of lymph nodes detected and the expression of CD44v6 and p27in cancer tissues after operation for colorectal cancer. Methods： 98 cases of patients who were diagnosed as colorectal cancer were selected as subjects. According to the random number table method,they were divided into combination group and controlled group,with 49 cases in each. The controlled group were treated with neoadjuvant chemotherapy before operation; the combination group were treated combined with oral administration of Tumor-suppressor Decoction before operation. The 5-year survival rate and quality of life in the two groups were compared. The number of lymph nodes detected and the expression of CD44v6 and p27in tumor tissues were recorded. Results： The number of detected lymph nodes and the KPS score in the controlled group at reexamination [（ 17. 2±2. 6）,（ 88. 9±3. 8） ]were significantly higher than those in the combination group than those in the controlled group [（ 10. 6±2. 3）,（ 85. 2±3. 7） ]. After treatment,the positive expression rate of CD44v6 protein in tumor tissues of the combination group（ 28. 6%） was significantly lower than that in the controlled group（ 53. 1%） while the positive expression rate of p27protein（ 77. 6%） was significantly higher than that in the controlled group（ 55. 1%）（ P 0. 05）. The 3- year survival rate in the combination group after operation（ 57. 1%） was significantly higher than that in the controlled group（ 34. 7%）（ P0. 05）. Conclusions： The application of Tumor-suppressor Decoction combined with neoadjuvant chemotherapy in the preoperative treatment of colorectal cancer can effectively improve the operation effect and promote the outcomes,which is good for the improvement of prognosis of patients.
