靳氏针刺法与综合康复锻炼治疗中风后痉挛性偏瘫临床研究

Clinical study of Applying JIN's Acupuncture Method and Comprehensive Rhabilitation Exercise in the Treatment of Post Stroke Spastic Hemiplegia

目的:观察靳三针刺法与综合康复训练对中风后痉挛性偏瘫患者临床治疗效果。方法:选取自2009年2月到2012年1月来我院就诊的中风后痉挛性偏瘫患者98例作为研究对象,按治疗方案的不同分为针刺组、康复组和混合组,针刺组31例采用单纯靳三针针刺治疗方案,康复组29例采用综合康复训练治疗方案,混合组38例采用靳三针针刺配合综合康复训练治疗方案,三组患者治疗期间对血压和血糖进行控制、调节血脂水平,并给予阿斯匹林和营养神经等药物常规治疗,分析三组患者神经功能恢复能力、综合康复能力、日常生活质量以及痉挛缓解状况。结果:在神经功能方面,混合组的疗效要好于针刺组和康复组(P<0.01),在综合康复能力评分(FCA)上,混合组要好于其余两组(P<0.05),在日常生活能力评分方面,三组之间两两比较无差异(P>0.05),在缓解痉挛程度方面,混合组要好于针刺组和康复组(P<0.05)。结论:靳三针配合康复训练的临床疗效要好于单用这两种方案,尤其是在缓解痉挛和恢复神经功能方面,临床可以作为治疗的一种参考样式。

Objective： To explore the effect of utilizing needle puncture and comprehensive rehabilitation training in stroke patients with spastic hemiplegia,provided a convenient,effective treatment scheme for the treatment of spastic hemiplegia after stroke. Methods： 98 cases of post stroke patients with spastic hemiplegia were selected as the research objects in our hospital from February 2009 to January 2012,according to the different treatment schemes for the acupuncture group,a rehabilitation group and mixed group,an acupuncture group of 31cases（ 31. 63%） treated by acupuncture method only,Jin＇s 3-needle therapy,rehabilitation group of 29cases（ 29. 59%） by comprehensive rehabilitation training in the treatment scheme,the mixed group 38cases（ 38. 78%） with Jin＇s three needle acupuncture combined with comprehensive rehabilitation training treatment,during treatment in the patients of three groups of control,regulating blood lipid level on blood pressure and blood sugar,and given aspirin and nutrition nerve routine medical treatment,analysis of three groups of patients with neurological function recovery ability,comprehensive rehabilitation ability,the quality of daily life,and relieving spasm of status. Results： In the neural function,the mixed group of curative effect was better than the acupuncture group and the rehabilitation group（ X = 11. 227,P = 0. 0040. 01）,scoring in the comprehensive rehabilitation capacity（ FCA）,mixed group as better than the other two groups（ t = 2. 162,2. 246,P =0. 034,0. 028 0. 05）,in the daily life ability score between the three groups,two- two comparison was no difference（ P 0. 05）,in relieving the spasm degree,mixed was better than the acupuncture group and the rehabilitation group（ X = 6. 132,P =0. 0470. 05）. Conclusion： The clinical curative effect of JIN＇s 3-needle combined with rehabilitation training is better than the single use of rehabilitation training on single use of the two kinds of schemes,especially in relieving spasm and the recovery of nerve function. Clinical can be used as a reference pattern treatment.
