摘要
目的 探讨超声实时弹性成像技术鉴别膝关节积液与滑膜增生的价值。方法 对68例MRI检出的同时有滑膜增生和关节积液的患者行超声弹性成像检查,依据弹性成像结果予以弹性分级(Ⅰ~Ⅳ),比较二者的差异。结果共纳入68例患者的72个膝关节,膝关节积液弹性成像评分Ⅰ级32个,Ⅱ级40个,滑膜评分Ⅲ级67个,Ⅳ级5个。弹性成像可清楚观察滑膜及积液的范围及分界,所示积液量低于二维超声所见。结论 超声弹性成像技术弹性评分法有助于膝关节积液与增生滑膜的鉴别诊断。
Objective To investigate the value of real-time ultrasound elastography in the differential diagnosis of e/fusion and synovial hypertrophy of knee-joint. Methods Totally 68 patients with knee-joint effusion and synovial proliferation di- agnosed by MRI were performed with ultrasound elastography. The difference of classification ( Ⅰ-Ⅳ ) according to the elastic image were compared. Results A total of 72 knee-joints from 68 patients were investigated. The elastic sore of knee-joint effusion ranked with Ⅰ and Ⅱ were 32 and 40 cases, respectively. While the elastic sore of synovial proliferation ranked with Ⅲ and Ⅳ were 67 and 5 cases, respectively, Ultrasound elastography could be used to clearly distinguish the scope and boundary of synovium and effusion, and the effusion volume was less than that detected by two-dimensional ul- trasound. Conclusion Ultrasound elaslography sore method has a value for the differential diagnosis of knee-joint effusion and synovial hypertrophy.
