浦东新区儿童保健门诊规范化服务现况研究

Standardized service of child health care outpatients in Pudong New Area: a crosssectional study

目的了解上海市浦东新区各级医疗机构儿童保健门诊规范化服务现状,查找存在问题,提出改进对策,提高服务能力。方法采用上海市妇幼保健中心统一制定的《儿童保健专业门诊用房、设备设施及人员调查问卷》,对全区50家儿童保健门诊进行现场调查。结果根据《上海市儿童保健门诊规范要求》规定,45家社区卫生服务中心儿童保健门诊房屋设置较5家二级及以上医疗机构儿童保健门诊房屋设置完善,但均存在设施设备欠缺问题。全区共配备儿童保健人员181人,以临床医师为主、专职为主;社区卫生服务中心儿童保健人员以初级职称为主,二级及以上医疗机构儿童保健人员以中高级职称为主。均实行门诊预约制,绝大多数儿童保健门诊存在超量服务现象。结论卫生行政部门及各级医疗机构应重视儿童保健门诊的规范化建设,完善网络建设与硬件设施建设,优化人员配置和业务培训,加强儿童保健健康教育,全面提高儿童保健服务能力。

Objective To understand the current situation of standardized service provided by medical institutions at all levels in Pudong New Area in Shanghai city, find the existing problems, put forward improvement countermeasures and service ability. Methods A uniform questionnaire about premises, facilities and personnel of professional child health care outpatients designed by Shanghai Maternal and Child Health Care Center were used to survey 50 child health care outpatients in Pudong New Area. Results According to child health care outpatient regulatory requirements in Shanghai city, child health care outpatients of 45 community health service centers have better housing setup compared with five higher level hospitals, but the former lacks facilities. There are 181 child health care doctors in the whole district, most of them are full-time clinicians. In community health service centers, most of clinicians have primary title, while most of them have intermediate or advanced titles in higher level hospitals. Outpatient appointment system is well performed in all clinics. The phenomenon of excess outpatient service is found in most institutes. Conclusion Health administrative departments and medical institutes should work together to build standardized child health clinics, improve network and facilities construction, optimize staff allocation and staff training, strengthen health education to improve child health care ability comprehensively.
