本文围绕延续护理的核心要素,介绍丹麦的脑卒中患者医院-社区-家庭的延续护理现状和支撑它的医疗卫生服务体系,从而提出对实现我国脑卒中患者医院-社区-家庭延续护理模式的启示。

This paper focused on the core elements of transitional care, and introduced the current status of the Hospital-Community-Continuity of Care for patients with stroke and health service system in Denmark. On this basis, the methods for transitional care in China were proposed.
