宫腔镜与腹腔镜手术剔除子宫壁间肌瘤的疗效对比

Curative effect of hysteroscopy and laparoscopy operation in treatment of uterine intramural myoma

目的:探讨宫腔镜与腹腔镜手术剔除子宫壁间肌瘤的临床疗效。方法:选取本院2011年12月至2013年12月诊治的子宫壁间肌瘤患者88例,采用随机数字表法分为两组,44例患者采用腹腔镜手术治疗为对照组,44例患者采用宫腔镜手术治疗为观察组,比较两组患者的治疗效果。结果:观察组患者手术时间、术中出血量、术后住院时间、术后首次妊娠时间均明显少于对照组,围手术期血红蛋白降低程度明显小于对照组,术后肛门排气时间、首次下床活动时间、术后肌层完全愈合时间均明显短于对照组,术后镇痛率、术后并发症发生率均明显低于对照组,术后妊娠率明显高于对照组,差异均有统计学意义(P<0.05)。结论:宫腔镜与腹腔镜手术均是治疗子宫壁间肌瘤的有效方法,其中宫腔镜手术的治疗效果更佳,对患者创伤小,可明显缩短患者的治疗时间和恢复时间,患者预后好且安全性高,值得临床推广使用。

Objectives： To investigate the clinical effect of hysteroscopy and laparoscopy operation in treatment of uterine intramural myoma. Methods： 88 patients with uterine intramural myoma were selected in hospital from December 2011 to December 2013 and randomly divided into two groups,44 patients treated by laparoscopy operation as control group and 44 patients treated with hysteroscopy operation as observation group. Therapeutic effect was compared between the two groups. Results： Operation time,intraoperative bleeding volume,postoperative hospitalization time,postoperative first pregnancy time in observation group were significantly less than control group. Reduction level of hemoglobin during operation in observation group was significantly smaller than control group. Postoperative anal exhaust time,first bed activity time,postoperative muscle layer completely healing time in observation group was significantly earlier than control group. Postoperative analgesia rate,incidence of postoperative complications in observation group was significantly lower than control group. Postoperative pregnancy rate in observation group was significantly higher than control group. All differences were statistically significant（ P〈0. 05）. Conclusions： Hysteroscopy and laparoscopy operation are effective methods in treatment of uterine intramural myoma. Hysteroscopy operation has better treatment effect,which has small trauma and can significantly shorten treatment time and recovery time with good prognosis and high safety,thusly is worthy of clinical use.
