祛瘀强骨汤联合西药治疗强直性脊柱炎临床研究

Clinical Observation on Quyu Qianggu Decoction Combined with Western Medicine in Treating Ankylosing Spondylitis

目的 :观察祛瘀强骨汤联合西药治疗强直性脊柱炎(AS)瘀血痹阻证的临床疗效。方法:将符合纳入标准的60例强直性脊柱炎患者,随机分为合治组和对照组各30例。对照组采用西药常规治疗,合治组在对照组的基础上加用祛瘀强骨汤治疗,连续治疗12周,观察治疗前后主要中医症状、AS病情活动指数(BASDAI)、功能指数(BASFI)、C反应蛋白(CRP)、血沉(ESR)等指标,统计疗效。结果:合治组总有效率86.57%,对照组67.51%,两组疗效比较差异有统计学意义(P<0.05)。两组治疗前后在改善腰骶疼痛、脊背疼痛、晨僵、刺痛、夜间疼痛等中医症状及中医证候评分、BASDAI、CRP、ESR方面差异有统计学意义(P<0.05);与对照组比较,合治组用药后在改善腰脊活动受限、口干不欲饮、肌肤干燥少泽及BASFI评分方面差异有统计学意义(P<0.05)。结论:祛瘀强骨汤联合西药能够缓解AS症状,降低ESR、CRP等炎性指标,疗效确切。

Objective：To observe the clinical effects of Quyu Qianggu decoction combined with western medicine in the treatment of ankylosing spondylitis of obstruction by blood stasis syndrome. Methods：Total 60 patients with ankylosing spondylitis were randomized into the treatment group（30 eases） and the control group（30 eases）. The control group was treated with routine western medicine,the treatment group was added with Quyu Qianggu decoction on the basis of the control group,for 12 weeks. The main TCM syrup toms,bath ankylosing spondylitis disease activity index（BASDAI）,bath ankylosing spondylitis functional index （BASFI）,C-reaction protein（CRP） and erythrocyte sedimentation rate（ESR） were observed before and after treatment. Results：The total effective rate was 86.57% in the treatment group and 67.51% in the control group,with statistical significance（P〈0.05）. TCM symptoms,TCM syndrome score,BASDAI,CRP and ESR had been improved after treatment in the two groups,with statistical significance（P〈0.05）. Compared to the control group,the activity limitation of the waist and skin and BASFI were improved in the treatment group ,with Quyu Qianggu decoction combined with western medicine spine,dry mouth with no desire to drink,dry statistical significance （P〈0.05）. Conclusion ： could relieve the symptoms of ankylosing spondylitis,reduce the inflammatory indexes of ESR and CRP with definite efficacy
