小承气肠灌肠联合针灸治疗急性胰腺炎疗效观察

Therapeutic effect of Xiaochengqi decoction combined acupuncture on early postoperative inflammatory intestinal obstruction

目的探讨加味小承气汤联合针灸治疗在治疗急性胰腺炎患者中的价值。方法选择2009年9月～2014年9月中山大学附属第五医院普外科住院治疗的78例急性胰腺炎患者．按随机原则均分为西医组、小承气汤组和联合治疗组。西医组给予常规西医对症处理，小承气汤组在西医常规治疗基础上给予中药小承气汤500mL保留灌肠，均每日2次。联合治疗组在小承气汤组治疗基础上针刺双侧足三里、上巨虚及下巨虚穴，每日2次，共治疗14d。观察术后患者肠道功能恢复情况、术后并发症发生情况。结果小承气汤组和联合治疗组总有效率分别为84．62％和92．31％，明显优于西医组69．23％。小承气汤组和联合治疗组腹痛、体温恢复、恢复排便排气时间及血淀粉酶恢复时间均优于西医组（P〈0．05）。结论采用小承气汤联合针灸治疗能明显促进急性胰腺炎患者的恢复，达到减轻患者痛苦，早日康复的目的。

Objective To explore Xiaochengqi decoction combined acupuncture on early postoperative inflammatory intestinal obstruction. Methods A retrospective analysis was conducted on a total of 78 acute pancreatitis patients who were admitted at the hospital in between September, 2009 and September, 2014. The patients were divided into three groups, a western medicine control group, a Xiaochengqi decoction group, and a combined treatment group, with 26 patients of each group. The Xiaochengqi decoction group was given a modified Xiaochengqi decoction 500 mL retention enema administration for twice a day, while the combined treatment group was given acupuncture therapy at the points of Zusanli, Shangjuxu, and Xiajuxu twice a day for a period of 14 days, on top of the treatment Xiaochengqi decoction group received. The intestinal function and postoperative complication were observed and compared. Results The total effectiveness of the Xiaoehengqi decoction group and the combined treatment group reflected 84.62% and 92.31%, obviously superior to 69.23% of the western medicine group. Abdominal pain, recovery time of body temperature, exhaust defecalion, and blood amylase of the two groups revealed better than the western medicine group （P〈0.05）. Conclusion Xiaochengqi decoction combined acupuncture treatment is considered being effective in shortening patients＇ recovery time and reducing patients＇ pain.
