WBC、hs-CRP、NLR联合检测在老年感染性疾病诊断中的应用

目的探讨白细胞(WBC)、超敏C-反应蛋白(hs-CRP)、中性粒细胞和淋巴细胞比值(NLR)联合检测在老年感染性疾病诊断中的应用。方法选择2015-02—2016-04大连市中山区人民医院收治的老年感染患者127例作为研究组,分为细菌感染组(n=67)、病毒感染组(n=60)。同时选择93例健康老年人群作为对照组。测定所有研究对象的WBC、hs-CRP和NLR。结果老年感染性疾病组WBC、hs-CRP和NLR均显著高于对照组,差异有统计学意义(P<0.05)。细菌感染组老年患者WBC、NLR和hs-CRP均显著高于病毒感染组,差异有统计学意义(P<0.05)。细菌感染组WBC、hs-CRP、WBC+hs-CRP、WBC+hs-CRP+NLR阳性率显著高于病毒感染组(P<0.001)。结论 WBC、hs-CRP和NLR三者联合检测有助于老年感染性疾病的诊断,特别适合于细菌性和病毒性感染的鉴别诊断。

Objective To explore the application of joint detection of white blood cell （WBC）, high sensitive C-reactive protein （hs-CRP）,neutrophil and lymphocyte ratio（NLR） in the diagnosis of senile infectious diseases.Methods 127 senile infected patients admitted to Zhongshan District People＇s Hospital in Dalian from February 2015 to April 2016 were taken as the research group, and they were divided into bacterial infection group（n=67） and virus infection group（n= 60）.At the same time, 93 healthy aged people were taken as the control group.WBC, hs-CRP, and NLR of all research objects were measured. Results Compared with the control group, the values of WBC, hs-CRP and NLR in the aged infectious disease group were significantly higher than those in the control group with differences of statistical significance（P 〈 0.05）.The values of WBC,hs-CRP and NLR in the bacterial infection group were significantly higher than those in the virus infection group with differences of statistical significance（P 〈 0.05）.The positive rates of WBC, hs-CRP, WBC ＋hs-CRP, and WBC＋hs-CRP ＋NLR in the bacterial infection group were significantly higher than those in the virus infection group（P 〈 0.001）.Conclusion Joint detection of WBC ,hs-CRP, and NLR can be helpful for the diagnosis of senile infectious diseases, especially for the differential diagnosis of bacterial infection and virus infection.
