两种预注射方式在冠状动脉CT成像中预防对比剂外渗的效果观察

Effect Observation of Two Pre-injection Methods in the Prevention Contrast Agent Extravasation in the Examination of Coronary CT Angiograph

目的探讨冠状动脉CT成像检查中两种预注射生理盐水的方式对预防对比剂外渗的效果,以期更有效地降低对比剂外渗发生率,提高CT检查的成功率。方法便利抽样选择南京大学医学院附属鼓楼医院2013年1月至2014年3月行冠状动脉CT检查的1982例患者为手动组,方法为注射对比剂之前使用手动旋钮注射生理盐水30 ml,速率约为1 ml/s;选择2014年4月至2015年6月行冠状动脉CT检查的2095例患者为高压组,方法为注射对比剂之前使用高压注射器以5.5 ml/s的速率注射生理盐水30 ml。比较两组患者对比剂的外渗发生率。结果手动组患者对比剂外渗率为0.80%(1例),而高压组仅为0.07%(16例),两组比较差异有统计学意义(X^2=14.150,P<0.001)。结论高压预注射生理盐水能更好地预防对比剂的外渗,更有效地保护患者血管,提高护理质量,值得临床推广应用。

Objective To investigate the effects of two ways of normal saline pre-injection to prevent the extravasation of contrast agent during the examinations of coronary CT angiography,in order to effectively reduce the contrast agent extravasation,thus to improve the success rate of CT examination.Methods One thousand nine hundreds and eighty-two patients who had been accept manual pre-injections of 30ml normal saline （1ml/s） before the injection of agent from January 2013 to March 2014 were collected as manual pre- injection group.The 2095 patients who had been accept high pressure pre-injections of 30ml normal saline （5.5 ml/s） before the injection of agent from April 2014 to June 2015 were collected as high pressure pre- injection group.The extravasation rates of contrast agent of two groups were compared.Results The ex- travasation rate of manual pre-injection group was 0.80% （1 case） while the other was 0.07% （16 cases） （x^2=14.150,P〈0.001）.Conclusions High pressure pre-injection is better for preventing the extravasation of contrast agent and protecting the patients ＇ vessels and it can improve the nursing quality, which is worth for clinical application.
