慢性胃炎中医证型及其症状条目库的构建和优化

Construction and Optimization of Chinese Medicine Syndrome and Symptom Items Database of Chronic Gastritis

目的基于文献检索结合人工评阅构建并优化慢性胃炎中医证型及症状条目库。方法收集相关组织机构制定的指南、共识及《中医内科学》中关于慢性胃炎的中医证型及症状,并检索中国知网、维普数据库、万方数据库、中国生物医学文献服务系统、中国中医药数据库及《证素辨证学》中与慢性胃炎相关的症状条目,初步构建慢性胃炎中医证型及症状条目库。采用仓储法、精选法、小组讨论法、专家定性评阅筛选、优化条目库,同时采用小组讨论法及专家定性评阅定义各中医证型及症状条目的证素归属。结果通过检索,初步构建的慢性胃炎中医证型条目库共计证型27个,筛选优化后共计证型14个;初步构建的慢性胃炎中医症状条目库共计症状365个,筛选优化后共计症状181个。慢性胃炎中医证型及症状的必备证素17个,其中病位证素5个(胃、脾、肝、大肠、小肠),病性证素12个(气虚、阳虚、阴虚、寒、外风、食积、痰、饮、湿、热、气滞、血瘀)。结论通过全面的文献搜索,采用仓储法、精选法、小组讨论法及专家定性评阅构建并优化了慢性胃炎的中医证型及症状条目库,形成包含症状条目181个、证型条目14个的慢性胃炎中医证型及症状条目库。

Objective To construct and optimize Chinese medicine syndrome and symptom items database of chronic gastritis based on literature reviewing combined with artificial marking. Methods Chinese medicine syndromes and symptoms of chronic gastritis were collected in guidelines and common view developed by related organizations, as well as internal medicine of Chinese medicine （中医内科学）. Symptom items related to chronic gastritis were reviewed in China National Knowledge Infrastructure, VIP Data, Wan Fang Data, Chinese Biological Medical Literature Database on disc, Chinese Traditional Medicine Database System and syndrome differentiation of syndrome element （证素辩证学）. Chinese medicine syndrome and symptom items database of chronic gastritis was constructed prelimina- rily. The items database was screened and optimized by storage method, selected method, group discussion method and qualitative expert marking and screening. Meanwhile, syndrome elements attribution of the Chinese medicine syndrome and symptom items was defined by group discussion method and qualitative expert marking. Results By reviewing, there were 27 syndrome types in total in preliminarily constructed Chinese medicine syndrome and symptom items database of chronic gastritis. After screening and optimizing, there were 14 syndrome types in total. There were 365 symptoms in total in preliminarily constructed Chinese medicine syndrome and symptom items database of chronic gastritis. After screening and optimizing, there were 181 symptoms in total. There were 17 essential syndrome elements of Chinese medicine syndromes and symptoms of chronic gastritis, including 5 focus syndrome elements （stomach, spleen, liver, large intestine and small intestine） and 12 disease character syndrome elements （Qi deficiency, Yang deficiency, Yin deficiency, cold, exogenous wind, food retention, phlegm, fluid retention, dampness, heat, Qi stagnation and blood stasis）. Conclusion Chinese medicine syndrome and symptom items database of chronic gastritis was constructed and optimized by comprehensive literature search. It was adopted that storage method, selected method, group discussion method and qualitative expert marking. Chinese medicine syndrome and symptom items database of chronic gastritis formed, which included 181 symptom items and 14 syndrome items.
