延续性护理干预对高血压患者的影响

Effect of Community Continuity Nursing Intervention on Blood Pressure Control,Quality of Life and Psychological Status of Patients with Hypertension

目的研究社区延续性护理干预对血压控制、生活质量、心理状态的影响。方法回顾分析自2012年7月至2015年7月以来,卫生队收治的97例高血压患者临床资料,依据护理方式将其分为延续性护理组(n=49)与常规护理组(n=48),常规护理组仅于卫生队治疗期间进行常规护理,延续性护理组在此基础上给予认知干预、用药干预、血压监测、生活干预、心理干预等社区延续性护理干预。观察对比两组干预前后血压控制情况,观察对比两组干预前后QOL(生活质量)、自我效能评分的改变情况,观察对比两组干预前后SAS(焦虑自评量表)、SDS(抑郁自评量表)、HAMA(汉密尔顿焦虑量表)、HAMD(汉密尔顿抑郁量表)等心理状态评分的改变情况。结果延续性护理组干预后收缩压、舒张压、QOL评分、自我效能评分、心理状态评分与常规护理组对比,均有统计学意义(P<0.05)。结论针对高血压患者实施社区延续性护理干预,通过提高长期自我管理能力,不仅可以稳定控制血压、改善生活质量,而且能够消除患者焦虑抑郁等负性情绪,效果显著,应予推广。

Objective To study the effect of hypertension community continuity nursing intervention on blood pressure control, quality of life and mental state. Methods a retrospective analysis of since July July 2012 to 2015, the medical team were 97 cases of hypertensive patients with clinical data, according to the mode of nursing will be classified as a continuation of the nursing group（ n = 49） and routine nursing group（ n = 48） and routine nursing group only in the health team during the treatment of routine nursing, the continuation of the nursing group on the basis of the given cognitive intervention, drug intervention, blood pressure monitoring, in- tervention, psychological intervention and other community continuity nursing intervention. Observation and comparison of two groups before and after the intervention of blood pressure control, and observe changes in QOL（quality of life） and the scores of self- efficacy compared between the two groups before and after interven- tion, and observe changes in SAS （self rating Anxiety Scale）, SDS （self rating Depression Scale） and HAMA （Hamilton anxiety scale） and HAMD （Hamilton Depression Scale （HAMD） and mental status score compared between the two groups before and after intervention. Results The continuity of nursing group after the inter- vention of systolic blood pressure, diastolic blood pressure, QOL score, self - efficacy score, psychological state score and the conventional nursing group were statistically significant（P 〈 0.05）. Conclusion For patients with hypertension and implementation of community continuity nursing intervention, by improving the long - term self management ability, can not only stable control of blood pressure, improve the quality of life, and to elimi- nate the anxiety and depression of patients with negative emotions and remarkable effect, which should be popu-larized.
