内镜下带膜金属支架治疗食管癌食管狭窄和食管气管瘘64例临床观察

Clinical Observation of Endoscopic Covered Stent for Treatment of 64 Cases with Malignant Esophageal Stenosis and Esophago Tracheal Fistula

目的应用带膜金属支架治疗中晚期食管癌食管狭窄及食管气管瘘,并观察其疗效。方法在电子内镜下对64例中晚期食管癌食管狭窄和食管气管瘘患者进行带膜金属支架置入术。结果 64例患者支架置入术后吞咽困难、呛咳症状明显改善,食管气管瘘患者瘘口全部闭合。术后最主要的并发症是胸痛,其余依次是反流性食管炎、出血、支架移位,大部分并发症经对症治疗后可获得缓解或消失。结论经内镜带膜金属支架置入术是治疗中晚期食管癌食管狭窄及食管气管瘘一种安全有效的方法,并发症经治疗后可得到明显缓解。

Objective To observe the effect of the covered stent in the treatment of advanced esophageal cancer with esophageal stenosis and esophago tracheal fistula. Methods Implanting the covered stents in 64 patients with advanced esophageal cancer stenosis and esophago tracheal fistula. Results The dysphagia and cough symptoms of 64 patients improved significantly. The main postoperative complications were chest pain,and the rest were anti reflux esophagitis,bleeding and stent migration. Most of the complications after symptomatic treatment can be relieved or disappeared. Conclusion Endoscopic coverd stent implantation is a safe and effective method in the treatment of advanced esophageal carcinoma of esophageal stenosis and esophago tracheal fistula,and the complications after treatment can ease.
