化学发光微粒子免疫法和反相高效液相色谱法测定环孢素A全血浓度的相关性

Study on Correlation of CMIA and RP-HPLC for Determination of Whole-Blood Cyclosporin A Concentration

摘要 目的 比较化学发光微粒子免疫(CMIA)法和反相高效液相色谱(RP-HPLC)法测定环孢素A(Cs A)全血浓度的相关性。方法 分别采用CMIA法和RP-HPLC法测定52份肾移植患者的Cs A全血样本,测定值采用Passing-Bablok法进行线性回归、用Bland-Altman法计算偏倚。结果 ARCHITECT环孢霉素测试与HPLC检验的平均差异偏倚为10.8 ng/m L(95%置信区间为1.5~20.24 ng/m L),CMIA测定值/HPLC测定值平均为1.19(95%置信区间为0.84~1.53),CMIA法测定结果平均高于HPLC20%左右。结论 CMIA法测定值均高于RP-HPLC法,用其进行Cs A血药浓度测定时应予以折算,并结合临床表现综合判断。 Objective To investigate the correlation of CMIA and RP - HPLC for the determination of whole - bloodcyclosporine A concentration. Methods 52 whole blood samples of renal transplant patients were assayed by CMIA and RP - HPLC and analysed by T test and linear regression analysis. Results The bias was 10.8 ng/mL(95% confidence interval was 1.5 ~ 20.24 ng/mL) between CIMA and HPLC. The relative value of CMIA/HPLC was 1.19 (95% confidence interval was 0.84 ~ 1.53 ) o Conclusion There is a trend for CMIA to give a higher value relative to HPLC.
