聚维酮碘冲洗联合普拉洛芬滴眼液预防白内障超声乳化术后感染及黄斑水肿的效果观察

The preventive effects of povidone iodine rinsing combined with pranoprofen ophthalmic solution on infections and macular edema of patients after receiving cataract phacoemulsification

目的探讨聚维酮碘冲洗联合普拉洛芬滴眼液预防白内障超声乳化术后感染及黄斑水肿的效果。方法选择接诊的80例白内障患者,随机分为观察组和对照组,观察组采用聚维酮碘冲洗联合普拉洛芬治疗,对照组采用单纯普拉洛芬治疗。观察2组患者治疗前后最佳矫正视力、眼压、前房闪辉值、黄斑中心凹厚度、眼部体征总积分变化,记录感染情况,并进行3个月的随访,观察黄斑水肿的发生率。结果治疗后,观察组最佳矫正视力优于对照组,差异有统计学意义(P<0.05);观察组前房闪辉值、黄斑中心凹厚度和眼部体征总积分均低于对照组,差异有统计学意义(P<0.05);观察组感染发生率为5.00%低于对照组的25.00%(P<0.05)。治疗后1周、治疗后1个月、治疗后3个月观察组黄斑水肿发生率均低于对照组,差异无统计学意义(P>0.05),但观察组疗效更为优异。结论聚维酮碘冲洗联合普拉洛芬可以有效预防白内障超声手术后感染和黄斑水肿的发生,有利于预后,值得临床应用推广。

Objective To observe the preventive effects of povidone iodine rinsing combined with pranoprofen ophthalmic solution on infections and macular edema of patients after receiving cataract phacoemulsification. Methods Eighty patients with cataract who were treated in our hospital were divided into observation group and control group according to random digits table. The patients in control group were treated by povidone iodine rinsing only,however,the patients in observation group were treated by povidone iodine rinsing combined with pranoprofen ophthalmic solution. The best corrected visual acuity,intraocular pressure,flare value of anterior chamber,the thickness of the anterior chamber,thickness of central fovea of macula,total scores of ocular signs and infection status were observed and compared between two groups before and after treatment,besides,the incidences of macular edema were observed after 3-month follow-up. Results After treatment,the best corrected visual acuity in observation group was superior to that in control group（ P 0. 05）. The flare value of anterior chamber,thickness of the macular and total scores of ocular signs in observation group were significantly lower than those in control group（ P 0. 05）. The incidence of infections in observation group was 5. 00%,which was significantly lower than that 25. 00% in control group（ P 0. 05）. The incidence of macular edema in observation group on 1w,1m,3m after treatment,respectively,which was lower than that in control group,but there was no significant difference between two groups（ P 0. 05）. Moreover the therapeutic effects in observation group were superior to those in control group. Conclusion The povidone iodine rinsing combined with pranoprofen ophthalmic solution can effectively prevent the occurrence of infection and macular edema after cataract phacoemulsification,which is beneficial to patient＇s prognosis,thus it is worth using widely in clinical practice.
