柏氏虎杖油膏对低位肛瘘术后创面愈合影响的临床研究

Clinical study on effect of Bai's Huzhang Ointment on postoperative wound healing of low anal fistulectomy

目的观察柏氏虎杖油膏对低位肛瘘术后创面愈合的影响。方法将150例低位肛瘘切除术后患者随机分为治疗A组、治疗B组与对照组,每组50例。治疗A组采用柏氏虎杖油膏纱布换药、治疗B组采用红油膏纱布换药,对照组采用凡士林纱布换药。各组均连续换药21天,观察比较术后创面愈合时间、每周创面愈合率、肛门疼痛及渗液症状分级的变化情况。结果 1治疗A组术后创面愈合时间显著早于治疗B组与对照组(P<0.05),而治疗B组与对照组创面愈合时间差异无统计学意义(P>0.05)。2换药第一周、第二周,治疗A组创面愈合率显著高于治疗B组、对照组(P<0.05);第三周时,治疗A组创面愈合率显著高于对照组(P<0.05),而与治疗B组比较差异无统计学意义(P>0.05)。3与本组术后第2天比较,各组第7天、第14天、第21天时肛门局部疼痛、渗液症状分级的差异均有统计学意义(P<0.05);术后第7天、第14天,治疗A组肛门创面疼痛、渗液分级明显轻于治疗B组、对照组(P<0.05)。结论柏氏虎杖油膏术后换药可显著加快低位肛瘘术后创面愈合,改善肛门局部疼痛、渗液症状。

Objective To observe the effect of Bai＇ s Huzhang ointment on wound healing after low anal fistuletomy. Methods 150 patients with low anal fistulectomy were randomly divided into the treatment group A, treatment group B and control group,50 cases in each group. The treatment group A was treated with Bai＇ s Huzhang ointment on the gauze. The treatment group B were treated with Hongyou ointment on the gauze. The control group was treated with vaseline on the gauze, with a course of 21 days. The wound healing progress, weekly wound healing rate, postoperative pain and exudate were observed and compared. Results ①The wound healing time in the treatment group A was significantly less than that in the treatment group B and control group （ P 〈 0.05 ）. There was no statistically significant difference between the treatment group B and control group （ P 〉 0.05 ）. ②In the first two weeks, the wound healing rate in the treatment group A was significantly higher than that in the treatment group B and control group （ P 〈 0.05 ）. In the third week, the wound healing rate in the treatment group A was significantly higher than that in the control group（ P 〈 0.05 ）, but the difference of wound healing rate between treatment group A and B showed no statistical significance （P 〉 0.05 ）. ③Compared within group after operation, the pain and exudate grading on the 7th, 14th ,21 th day were statistically significant compared with those on the 2nd day （P 〈 0.05 ）. The pain and exudate grading in the treatment group A were significantly lighter than those in the treatment group B and control group on the 7th and 14th day after operation （ P 〈 0.05 ）. Conclusion Bai＇ s Huzhang ointment could obviously accelerate wound healing and improve pain and exudate after low anal fistulectomy.
