摘要
目的 探讨超声在甲状腺腺瘤与结节性甲状腺肿中的图像特征及诊断准确性,分析超声对两者诊断及鉴别的应用价值。方法 选取2014年1月~2015年12月我院120例经过手术及病理诊断为甲状腺腺瘤和结节性甲状腺肿的患者,各60例,根据诊断结果不同分为甲状腺腺瘤组与结节性甲状腺肿组,采用彩色多普勒超声进行检查,观察两组患者甲状腺病变的大小、形态、数量、包膜与周围组织关系及血流信号,比较两组影像学特征。结果结节性甲状腺肿的诊断准确率为93.3%,甲状腺腺瘤的诊断准确率为90.0%。两组各个超声影像学特征比较,差异有统计学意义(P〈0.05)。两组超声血流信号分型比较,差异有统计学意义(P〈0.05)。结论 甲状腺腺瘤及结节性甲状腺肿各个超声影像学特征差别明显,超声血流信号分型可以帮助医生分析结节性甲状腺肿和甲状腺腺瘤的超声影像,有助于进行鉴别诊断,可为临床的诊断及治疗甲状腺疾病提供重要的依据。
Objective To compare the image features and the diagnostic accuracy of ultrasound in thyroid adenoma and nodular goiter so as to analyze the effectiveness of application of ultrasound in the diagnosis and differential diagnosis of the two diseases.Methods From January 2014 to December 2015 altogether 120 surgically- and pathologically-diagnosed thyroid adenoma and nodular goiter patients in our hospital (60 cases of patients for each disease) were selected and divided into two groups (thyroid adenoma group and nodular goiter group) according to the different diagnostic re- sults.Color doppler ultrasonic examinations of the two groups of patients were performed so as to observe the size,shape, quantity,capsule and surrounding tissue and blood flow signal of thyroid lesions and compare the image features of the two groups.Results The diagnostic accuracy rate of nodular goiter was 93.3%,and the diagnostic accuracy rate of thyroid adenoma was 90.0%.The ultrasound imaging features of the two groups were significantly different ( P〈O.05).The distribution of ultrasonic blood flow signal classification of two groups showed obvious significant differences (P〈0.05). Conclusion Various ultrasound image characteristics of nodular goiter and thyroid adenoma are significantly different. Doppler ultrasound blood flow signal classification can help doctors to analyze the ultrasound images of thyroid adenma group and nodular goiter group, which is also helpful for differential diagnosis of the two diseases and can provide an important basis for the clinical diagnosis and treatment of thyroid diseases.
