摘要
目的:探讨代谢综合征(MS)对良性前列腺增生(BPH)患者临床进展的影响。方法:回顾性分析BPH患者241例,分析MS及其各组份包括:体质量指数(BMI)、腰臀比(WHR)、腰围(WC)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL-C)和低密度脂蛋白(LDL-C)与BPH之间的相关性。结果:在BPH患者临床进展分组中低进展组(LPG)、低进展组-1(LPG-1)、低进展组-2(LPG-2)和高进展组(HPG)患者的SBP、TG、TC、HDL-C、LDL-C检验差异有统计学意义(P<0.05);前列腺体积(PV)与TG、TC、LDL-C呈正相关,与HDL-C呈负相关(P<0.05);国际前列腺症状评分(IPSS)与年龄、WHR、SBP、DBP呈正相关(P<0.05);生活质量评分(QOL)与WHR、DBP、SBP呈正相关(P<0.05);最大尿流率(Qmax)与年龄呈负相关(P<0.05);年龄、WHR、SBP与储尿期症状、排尿期症状、排尿后症状均呈正相关(P<0.05);DBP与储尿期症状呈正相关(P<0.05)。结论:MS及其组份是诱发BPH的危险因素,且能够促进BPH的发生及临床进展。
Objective To investigate impact of metabolic syndrome (MS) on clinical progression of patients with benign pros-tatic hyperplasia (BPH). Methods: 241 patients in the urinar^^ department of Jilin central hospital were collected, divided into low progression group (LPG) , low progression group -1 ( LPG -1), low progression group -2 ( LPG -2) and high progression group (HPG) , and retrospectively analyzed. The correlations among BPH and MS and its components like body mass index (BMI) , waist- to-hip ratio (WHR) , waist circumference (WC) , systolic blood pressure ( SBP) , diastolic blood pressure ( DBP) , fasting plasma glucose (FPG) , triglyceride (TG) , total cholesterol (TC) , high-density lipoprotein (HDL-C) , and low-density lipoprotein (LDL- C) were analyzed. Results: There were statistical differences in the SBP, TG, TC, HDL-C and LDL-C among the LPG, LPG-1 , LPG-2 and HPG ( P〈0. 05). The prostate volume (PV) was positively correlated with TG, TC and LDL-C, and negatively correlated with HDL-C (P〈0.05). The international prostate symptom score (IPSS) was positively correlated with age, WHR, SBP and DBP (P〈0. 05). The quality of life ( QOL) score was positively correlated with WHR, SBP and DBP (P〈0. 05 ). The Qmax was negatively correlated with age ( P 〈0 . 05). The voiding stage symptom (VSS) , storage stage symptom (SSS) and pos tvoid stage symptom (PSS) were positively correlated with age, WHR and SBP (P〈0. 05). The SSS was positively correlated with DBP (P〈0. 05). Con-clusions :MS and its components are the risk factors of BPH, and can promote the development and clinical progression of BPH.
