摘要
目的探究老年慢性牙周炎合并冠心病患者龈沟液及血清中白细胞介素8(IL-8)、血清肿瘤坏死因子α(TNF-α)、白细胞介素10(IL-10)表达水平,研究慢性牙周炎与冠心病的相关性。方法选择2013年2月至2015年4月期间治疗的老年患者76例作为研究对象,其中A组为慢性牙周炎合并冠心病患者(40例),B组为单纯患有慢性中重度牙周炎患者(36例)。同期健康对照组为C组(38例)。对三组患者探诊深度、探诊出血指数等指标进行记录。所有患者均抽取龈沟液及肘正中静脉血,采用酶联免疫吸附法(ELISA)对IL-8、TNF-α、IL-10水平进行分析,探究慢性牙周炎与冠心病相关性。结果 A、B组患者牙周指数BI、PD等指标显著高于C组健康志愿者,A组指标高于B组,B组指标高于C组,表明三组患者牙周炎严重程度顺次降低,差异有统计学意义(P<0.05)。A组、B组被试的龈沟液及血清IL-8、TNF-α水平均显著高于C组,A组、B组龈沟液、血清IL-10水平均显著低于C组,差异有统计学意义(P<0.05);A组的龈沟液及血清IL-8、TNF-α均显著高于B组,IL-10水平低于B组,差异均有显著性(P<0.05)。牙周指数与IL-8、TNF-α水平呈正相关关系(P<0.05),牙周指数与血清、龈沟液内IL-10水平呈负相关关系(P<0.05)。结论老年慢性牙周炎与冠心病间存在相关性,它们有共同的致病因素,与血清IL-8、TNF-α、IL-10等炎性因子有一定联系。
Objective To explore the expression level of interleukin 8 (IL - 8 ),TNF - α、interleukin 10 (IL - 10) of elderly patients with chronic periodontitis and coronary heart disease in patients with gingival crevicular fluid and serum, and the relationship between chronic periodontitis and coronary heart disease were studied. Methods During February 2013 to April 2015 , 76 elderly patients were studied as research subjects, A group patients with chronic periodontitis combined with coronary heart disease (40 cases) , B group only suffering from chronic peri-odontitis patients (36 cases) , group C was control group (38 cases). Probing depth, bleeding on probing index in the group A, B and C were re-corded. Gingival crevicular fluid and cubital vein blood were abstracted, IL-10, TNF - α , IL - 8 level were analyzed using enzyme - linked immunosorbent assay (ELISA) to explore relationship between chronic periodontitis and coronary heart disease. Results Periodontal index BI, PD index of group A and B were significantly higher than those of group C and healthy volunteers. Those of group A was highest and those of group C was the lowest, the difference was statistically significant ( P 〈0. 0 5 ) . GCF and serum IL - 8 , TNF - α levels in group A and group B were sig-nificantly higher than those in group C, gingival sulcus fluid, serum IL-10 levels in group A and B were significantly lower than those of group C, the difference was statistically significant ( P 〈 0. 0 5 ) . Periodontal Index and IL - 8 , TNF - α levels were positively correlated ( P 〈 0. 05 ) , periodontal index and serum, IL - 10 levels in gingival were negative correlation ( P 〈 0. 05 ) . Conclusion There was a correlation between chronic periodontitis and coronary heart disease in elderly patients, they have common pathogenic factors, and the incidence of periodontitis and coronary heart disease and IL - 8 , IL-10 levels were correlated.
