“病房-门诊-家庭”三位一体全程护理模式在慢性阻塞性肺疾病患者综合护理中的应用

The application of ＆quot;ward-clinic-family＆quot; three-in-one whole-course nursing mode on comprehensive nursing for patients with chronic obstructive pulmonary disease

目的建立有效的慢性阻塞性肺疾病（ COPD）患者“病房-门诊-家庭”三位一体全程护理模式并评价其效果。方法选取180例慢性阻塞性肺疾病患者作为研究对象,随机均分为实验组（90例）与对照组（90例）,对照组采用常规护理,实验组在常规护理的基础上实施“病房-门诊-家庭三位一体”全程护理模式,比较两组护理前、后24周肺功能指标变化、BODE指数、护理满意度、生存质量、急性加重次数、住院费用以及不良反应发生率等差异。结果“病房-门诊-家庭”三位一体全程护理模式组患者肺功能指标变化、生存质量评分、护理满意度、自我护理能力方面明显优于对照组,差异有统计学意义,BODE指数、急性加重次数以及住院费用、不良反应发生率等明显低于对照组,差异有统计学意义。结论“三位一体全程护理模式”能有效地提高COPD患者的生存质量及护理满意度,改善患者的生存质量,在临床综合护理中值得进一步的推广。

Objective To establish an effective＂ ward-clinic-family＂ three-in-one whole-course nursing mod-el for chronic obstructive pulmonary disease （ COPD） patients and evaluate it＇s effects. Methods A total of 180 cases of chronic obstructive pulmonary disease patients were selected as the research objects and randomly divided into an ex-perimental group and a control group, 90 cases in each group. The control group were treated with conventional care, while the experimental group received＂ward-clinic-family＂ three-in-one whole-course nursing model on the basis of rou-tine nursing care. The differences of the changes of lung function index, BODE index, nursing satisfaction, quality of life, times of acute exacerbation, hospitalization expenses and the incidence of adverse reactions of the two groups before and after 24 weeks＇ nursing were compared. Results The changes of lung function index, the score of life quality, nurs-ing satisfaction and self-care ability of the experimental group were superior to that of the control group, and the differ-ences were statistically significant （P〈0. 05） . The BODE index, times of acute exacerbation, hospitalization expenses and the incidence of adverse reactions of the experimental group were significantly lower than that of the control group, and the differences were statistically significant （ P〈0. 05 ） . Conclusions ＂Three-in-one whole-course nursing model＂can improve the life quality and nursing satisfaction of patients with COPD effectively, and improve the quality of life of patients. It is worthy of further promotion in clinical comprehensive nursing.
