摘要
目的:探讨胰岛素泵治疗2型糖尿病在围手术期的临床应用效果。方法:前瞻性选取2014年5月-2015年7月笔者所在医院内分泌科收治的30例2型糖尿病患者,采用投掷硬币法将其分为两组,每组15例,对照组采用常规皮下注射胰岛素,观察组采用胰岛素泵皮下注射胰岛素,比较两组患者的血糖控制情况、胰岛素用量及围手术期低血糖发生情况。结果:两组患者经治疗后空腹血糖和餐后2 h血糖水平均明显低于治疗前,差异均有统计学意义(P<0.05),且观察组降低程度均较对照组更为明显,差异均有统计学意义(P<0.05);观察组血糖达标时间明显短于对照组,胰岛素用量明显少于对照组,差异均有统计学意义(P<0.05);观察组围手术期低血糖发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:胰岛素泵治疗糖尿病为患者度过围手术期提供了安全、有效、可靠的治疗方法,值得临床推广。
Objective: To investigate the clinical effect of insulin pump in the perioperative period treatment of type 2 diabetes.Method: Thirty patients with type 2 diabetes admitted to our hospital from May 2014 to July 2015 were selected and divided into two groups, 15 cases in each group.The control group was given subcutaneous injection with insulin conventionally, while the observation group was subcutaneous injected by the insulin pump.The blood glucose control, insulin dosage and perioperative hypoglycemia were compared between the two groups.Result: The fasting blood glucose and postprandial 2 h blood glucose levels after treatment in both of the two groups were significantly lower than before treatment, the differences were statistically significant(P〈0.05), and which of the observation group was reduced more obviously than the control group, the differences were statistically significant(P〈0.05).The time of blood glucose control in the observation group was significantly shorter than that in control group, and the insulin dosage of which was significantly less than that of the control group, the differences were statistically significant(P〈0.05).During the perioperative period, the incidence of hypoglycemia of the observation group was significantly lower than that of the control group, the difference was statistically significant(P〈0.05).Conclusion: Insulin pump therapy provides a safe, effective and reliable treatment method for patients with diabetes to spend the perioperative period, it is worth clinical promotion.
